摘 要:目的:观察Sox9基因在人颈椎椎体终板软骨细胞退变模型中的表达变化,探讨其在人颈椎终板软骨细胞退变过程中的作用及意义。方法:选取2011年7~11月在皖南医学院附属弋矶山医院脊柱外科住院接受前路椎体次全切手术的颈椎病患者和颈椎骨折或脱位患者30例,分为对照组(12例)和颈椎病组(18例),建立人颈椎椎体终板软骨细胞退变模型。运用RT-PCR法检测终板软骨细胞退变过程中Sox9、Ⅱ型胶原及蛋白多糖基因的表达变化。结果:对照组原代终板软骨细胞大部分呈圆形,而颈椎病组原代终板软骨细胞呈梭形为主;与对照组相比,Sox9基因mRNA的表达明显下降(t=17.973,P<0.01),受其调控的蛋白多糖及Ⅱ型胶原基因基因mRNA的表达均相应减低(t=15.685、9.264,P值均<0.01);Sox9基因mRNA与蛋白多糖基因及Ⅱ型胶原基因mRNA的表达均存在正相关(r=0.976、0.958,P值均<0.01)。结论:Sox9基因在人颈椎终板软骨细胞退变过程中可能起着重要的作用,调节Sox9基因在人颈椎终板软骨细胞中的表达可能是治疗人颈椎椎间盘退变的一条新途径。Objective:To observe the in vitro change of Sox9 expression in chondrocytes in the degenerate model of human cervical vertebrae-endplate and investigate the significance of the expression variation in degeneration process of cervical vertebral endplate chondrocytes.Methods:Cartilage endplates were obtained from 30 inpatients,admitted to our hospital between Jul.and Nov.2011,for developing the degenerative models,and these patients were divided into control group(n=12) with cervical vertebral fracture or dislocation and cervical spondylosis group(n=18) with cervical spondylotic myelopathy.Endplate chondrocytes were isolated by enzyme digestion and cultured in vitro for observing the morphological appearances by normal and degenerative cartilage endplates with inverted phase contrast microscope.RT-PCR was used to detect the mRNA expression of Sox9,aggrecan and typeⅡcollagen.Results:Morphologically,the primary endplate chondrocytes in the control group appeared mostly polygons and fusiform in the cervical spondylosis group.As compared with the control group,the mRNA expression of Sox9 was significantly decreased(0.199±0.034 vs.0.553±0.027;t=17.973;P〈0.01),and the mRNA expression level of aggrecan and typeⅡcollagen,regulated by Sox9 gene,was decreased correspondingly(0.429±0.033 vs.0.790±0.045;0.533±0.025 vs.0.721±0.043;t=15.685,9.264;P〈0.01,respectively).The mRNA expression of Sox9 was positively correlated with the mRNA expression of aggrecan and type Ⅱ collagen(r=0.976,0.958;P〈0.01,respectively).Conclusion:Sox9 gene may play a role in the process of degeneration of cervical endplate chondrocytes.Regulating the expression of Sox9 gene in cervical endplate chondrocytes may be a new approach to treat the degeneration of cervical intervertebral disc.
