InterTan与DHS治疗股骨转子间骨折的临床对照研究

摘　　要：目的对比分析采用InterTan和动力髋螺钉治疗股骨转子间骨折的疗效。方法回顾性分析2009年1月至2010年10月收治的36例股骨转子间骨折患者的临床资料。采用InterTan治疗股骨转子间骨折13例,男7例,女6例,年龄59～87岁,平均73.5岁;采用动力髋螺钉治疗股骨转子间骨折23例,男14例,女9例,年龄28～85岁,平均71.2岁。观察手术时间、术中出血量、术后引流量、骨折临床愈合时间、术后并发症及术后末次评分。术后髋关节功能按Harris评分评定。结果 36例均获随访,时间5～1 3个月,平均7.2个月。在手术时间、术中出血量、术后引流量及Harris评分方面,InterTan组优于DHS组;在术后并发症和愈合时间方面,两组间无统计学差异。结论 InterTan操作简单、创伤小、固定牢靠、并发症少,是治疗股骨转子间骨折的有效方法。Objective To analyze and compare the clinical effects of InterTan and DHS in the treatment of In-tertrocbanteric fractures. Methods From January 2009 to October 2010,totally 36 cases with intertrochanteric frac-tures were treated with InterTan and DHS retrospectively,and the condition of intra-operation and postoperative re-covery were analyzed. 13 patients,including 7 males and 6 females with the age of 59 to 87 years old （average 73.5 years old）,were treated with InterTan ;and the other 23 patients ,including 14 males and 9 females with the age of 28 to 85 years old （average 71.2 years old）,were treated with DHS. Operative time,blood loss ,drainage,healing time, complications ,and Harris scores were collected for the statistical analysis. Results All patients were followed up for 5 to 13 months （averaged 7.2 months）. Statistically,InterTan group had better effects than DHS group on following aspects,such as operative time,blood loss ,drainage and Harriss score. There was no significant difference in healing time and complications between two groups. Conclusion InterTan is simple,minimally invasive,stable fixation with fewer complications,and is an effective method for intertrochanterie fractures.
