剖腹产术后舒芬太尼单次硬膜外镇痛的时间观察

摘　　要：目的探讨剖腹产术后单次硬膜外注射舒芬太尼的镇痛时间。方法60例择期硬膜外麻醉下行剖腹产患者，年龄20-35岁，ASAⅠ～Ⅱ级，以2％利多卡因麻醉，手术缝皮时，患者随机分为两组，舒芬太尼组（n=30）硬膜外腔给予舒芬太尼20ug（5ml），对照组（n=30）给予生理盐水（5ml）。观察患者第一次要求镇痛时间和不良反应。舒芬太尼镇痛时间采用Kaplan．Meier法进行分析。结果舒芬太尼组和对照组镇痛时间中位数分别为178min（95％CI117～220）和40min（95％CI5-62）。结论剖腹产术后单次硬膜外注射舒芬太尼20岭的镇痛时间为178min。Objective To determine the analgesic duration of single-dose epidural sufentanil after cesarean section. Methods Sixty ASA Ⅰ-Ⅱ patients undergoing caesarean section （C/S） with epidural 2% lidocaine were ran- domized into two groups. Sufentanil group （n=30） received epidural administration of sufentanil 20μg, and the control group （n=30） received saline 5 ml after the C/S procedure. The analgesic duration and side effects were recorded. The du- ration of analgesia was defined as the time from sufentanil administration until the first request for supplemental analge- sia. Duration of epidural sufentanil analgesia was compared between the two groups using Kaplan-Meier survival analysis. Results The median duration of analgesia for subjects in sufentanil group and the control group was 178 min （95% CI 117-220） and 40 min （95% CI 5-62）, respectively, which is significantly longer in sufentanil group than the control group. Conclusion Single-dose epidural sufentanil （20 mg） after cesarean section results in longer duration of analgesia.
