血清瘦素、肿瘤坏死因子α及白细胞介素6与慢性阻塞性肺疾病缓解期患者营养不良的相关性

摘　　要：目的探讨血清瘦素(leptin)、肿瘤坏死因子α(TNF-α)、白细胞介素6(IL-6)与慢性阻塞性肺病(chronicobstructive pulmonary disease,COPD)缓解期患者营养状况的关系。方法测定50例COPD缓解期男性患者(营养不良组25例、非营养不良组25例)及25例正常人的leptin、TNF-α、IL-6水平和理想体质量百分比(NBM%)、体质量指数(BMI)、三头肌皮褶厚度(TSF)、上臂肌围(AMC)、血清白蛋白(ALB)等营养学指标,并分析leptin、TNF-α、IL-6水平与营养学指标的相关性。结果 COPD营养不良组TNF-α及IL-6水平均明显高于COPD非营养不良组(P<0.01)。COPD营养不良组leptin水平明显低于COPD非营养不良组,(4.74±0.92)μg/L vs(6.96±0.75)μg/L(P<0.01)。3组血清leptin浓度与NBM、BMI、TSF、AMC等营养学指标均有明显正相关(P<0.01)。COPD营养不良组血清TNF-α浓度与NBM、BMI正相关性(P<0.05),与余各指标均无相关性。3组血清IL-6浓度与营养学指标、leptin及TNF-α均无相关性。结论 leptin与COPD缓解期患者营养学指标有明显相关性。Objective To observe relationship between malnutrition and serum leptin, tumor necrosis factor-a and interleukin-6（IL-6） levels in patients with stable chronic obstruction pulmonary disease（COPD）. Methods Serum leptin,TNF-a and IL-6 concentrations of 50 COPD patients in stationary phase（divided into two groups：25 patients in malnutrition group;25 patients in non malnutrition group） and 25 healthy volunteers were tested by ELISA. The ideal body mass percentage （ NBM%）, body mass index （BMi）, triceps skin-fold thickness （ TSF）, mid-upper arm circumference（MAC） and serum albumin（ALB） were measured in all patients. The correlation between leptin,TNF-a, IL-6 concentrations and nutritional parameters was analyzed. Results Serum TNF-a and IL-6 concentrations in malnutrition patients were evidently higher than in non malnutrition patients. The letin level in malnutrition patients was significantly lower than in non malnutrition patients, （4.74 ± 0.92） ug/L vs （ 6.96 ± 0.75） ug/L （ P 〈 0.01 ）. Serum leptin concentrations were significantly positive correlated with some nutritional parameters such as NBM%, BMI,TSF,AMC in all three groups（ P 〈0.01）. Serum TNF-a concentrations were positive correlated with NBM% and BMI in malnutrition patients （ P 〈0.05）, while serum TNF-a concentrations were not correlated with other parameters. Serum IL-6 concentrations were not correlated with serum leptin, TNF-a concentrations and nutritional parameters. Conclusion Serum leptin concentration was significantly correlated with nutritional parameters in COPD patients in stationary phase.
