体外循环心内直视先心纠治术对心肌肌钙蛋白的影响及意义

摘　　要：目的 :研究心脏不停跳心内直视先心纠治术对心肌肌钙蛋白 (c Tn- T)的影响及意义。方法 :检测 16例体外循环心脏不停跳心内直视先心纠治术治疗患者术前、术中及术后血清 c Tn- T及肌酸磷酸激酶同功酶 (CK- MB) ,并进行对比分析。结果 :血清 c Tn- T明显增高 ,于停机时达到最高峰 ,并持续高于术前水平至术后 2周。CK- MB于术后 2 h达最高峰 ,术后 1周即恢复至术前水平。结论 :体外循环心脏不停跳先心纠治术中 ,血清 c Tn- T可用于评价围手术期心肌细胞损伤和心肌保护效果的指标 ,其敏感性、特异性及持续时间优于 CK- MB。Objective:To study the effects and significance on cardiac troponin T(cTn T) during cardiac surgery with compulmonary bypass (CPB) in patients with congenital heart disease (CHD) Methods:During the perioperative term,the serum concentration of cTn T and creatine kinase MB(CK MB) were measured in 16 patients with CHD who had undergone cardiac surgery with CPB Results:cTn T came to its peak while stopping CPB and kept a higher level than preoperative level for two weeks postoperatively,however,CK MB reached its peak in 24 hours post operative course and returned to its preoperative level for 7 days postoperatively Conclusion:cTn T may be an useful and valuable indicator in evaluating the degree of myocardial cell damage and myocardial protection,and its censitivity,specificity and lasting time are better than cardiac erzymes
