摘 要:目的探讨2型糖尿病合并尿路感染患者的常见病原菌,并对耐药性及危险因素进行分析,为临床诊治提供依据。方法回顾性分析我院2009年6月至2011年12月收治的确诊为2型糖尿病合并尿路感染患者的临床资料,统计分析中段尿细菌培养加药敏试验检查结果及危险因素。结果 2型糖尿病合并尿路感染发生率为25.80%,以革兰阴性菌为主,占80.47%(103/128),常见革兰阴性杆菌对亚胺培南、哌拉西林/他唑巴坦、阿米卡星敏感性高;常见革兰阳性菌对万古霉素、吗啉恶酮/利奈唑胺敏感性高,对环丙氟哌酸、红霉素的耐药性最高。年龄较大、女性患者、血糖偏高及病程长是尿路感染的危险因素,差异有统计学意义(P<0.05)。结论年龄、性别、血糖和病程是2型糖尿病合并尿路感染的易感因素;2型糖尿病合并尿路感染耐药性高,临床应按药敏结果合理选择抗生素。Objective To explore the common pathogens in type 2 diabetes with urinary tract infection, and provide the clinical diagnosis and treatment through analysis on the drug resistance and risk factors. Methods Clinical data of type 2 diabetes with urinary tract infection were retrospectively analyzed from June 2009 to December 2011 ; the middle of urine bacteria cultivation of drug sensitive experiment test results and risk factors were analyzed. Results Type 2 diabetes combined with urinary infection rate was 25.80%, mainly composed of gram-negative bacteria by 80.47% ( 103/128 ), common gram-negative bacteria were sensitive to imipenem, piperacillin/sulbactam, and amikacin ; common gram-positive bacteria were sensitive to vancomycin, linezolid/rina thiazole amine, and were mostly resisted to ciprofloxacin and erethremycin. Older age,female patients, high blood sugar and the long disease coursewere urinary infection risk factors, the difference was statistically significant( P 〈 0.05 ). Conclusion Age, gender, blood sugar and course of the disease are the susceptible factors of type 2 diabetes with urinary tract infection, which has high drug resistance, therefore antibiotics should be reasonably selected according to the drug sensitivity test results.
