直视下尿道内切开治疗尿道狭窄50例分析

摘　　要：目的:对直视下尿道内切开术(DVIU)的术后疗效进行分析,探讨减少尿道狭窄复发的措施。方法:回顾性分析2005年1月～2011年10月50例尿道狭窄行DVIU手术的临床资料,根据尿道狭窄长度分组,观察每组复发情况及保留导尿时间。结果:其中术前狭窄长度≤1.0 cm 28例,复发3例,1.1～2 cm 14例,复发5例,2～3 cm 5例,复发3例,≥3 cm 3例,复发2例。术后随访1年,其中术后3个月复发9例,术后半年4例。术后留置尿管时间1～8周。结论:选择≤1 cm的尿道狭窄作为手术最佳适应证,术中轻柔操作,合理选择留置尿管时间,坚持尿道扩张等,能降低DVIU术后再次尿道狭窄的复发率,复杂尿道狭窄应当有选择性地应用DVIU。Objective：To analyse the curative effect of the direct vision internal urethrotomy （DVIU） and to explore the measures to reduce the recurrence of urethral stricture. Methods：We analyse retrospectively the clinical date of the 50 cases from January 2005 to Octobor 2011 who was operated by the DVIU, divide into 4 groups according to the length of urethral stricture, and combine the recurrence and retention time of catheter with each team. Results：There are 28 cases whose stricture length 41.0 cm, in which 3 cases recurred. And there are 5 in 14 cases in team 1.1--2 cm, 3 in 5 cases in team 2--3 cm, 2 in 3 cases in team ~3 cm recurred. There are 9 cases recurred in 3 months after the operation. The time of retention catheter is about 1--8 weeks. Conclusion： Urethral stricture ≤1cm is the best indication for DVIU and correct length of catheter indwelling can reduce the reccurrence of secondary urethral stricture after DVIU;DVIU should be applied seletively.
