消化道吻合器和缝合器在消化道吻合时并发严重并发症的原因分析与处理

摘　　要：目的探讨消化道吻合器及缝合器在上消化道吻合的并发症及处理措施。方法回顾性分析某院自2008年1月～2010年1月收治的452例需行消化道吻合的老年患者,均给予消化道吻合及缝合,分析治疗临床效果,并发症及相应的处理措施。结果胃大部切除行胃-空肠吻合,吻合与缝合次数分别为139次及417次,时间为8.4 min;全胃切除行食管-空肠吻合,吻合与缝合次数分别为156次及312次,时间为10.6 min,近端胃大部切除吻合与缝合次数分别为139次及278次,时间为12.1 min。本组发生并发症10例,发生率为2.2%,吻合口狭窄4例,吻合口出血3例,术后吻合口瘘4例。结论消化道吻合器及缝合器在上消化道吻合中出现并发症与多种因素有关,适当处理可减少并发症的出现。OBJECTIVE To explore the reason and disposal of stapler and stitching instrument in digestive inosculans.METHODS We retrospective analyzed 452 old patients who had stapler and stitching instrument in digestive inosculans in our department from January 2008 to January 2010,452 cases were given stapler and stitching instrument,and their clinical RESULTS and complications were analyzed.RESULTS Subtotal gastrectomy were given stomach-jejunum inosculans,and the number of stapler and stitching instrument was 139,417.The time was 8.4 min.Total gastrectomy was given esophagus-jejunum inosculans,and the number of stapler and stitching instrument was 156,312.The time was 10.6 min.Proximal gastrectomy was given esophagus-stomach inosculans,the number of stapler and stitching instrument was 139,278,and the time was 12.1 min.The rate of complicate was 4.2%（19/452）,including Anastomotic stricture（8 cases）,anastomotic bleeding（6 cases）,and anastomotic leakage（5 cases）.CONCLUSION There are mutiple factors involving stapler and stitching instrument in digestive inosculans,and the treatment combining various way is effective for reducing complications.
