小切口非超声乳化白内障摘除术治疗极硬核白内障临床观察

摘　　要：目的观察小切口非超声乳化白内障摘除术在治疗极硬核白内障中的应用价值。方法极硬核白内障患者31例,37只眼接受小切口非超声乳化白内障摘除术治疗,记录手术过程、观察术中术后并发症、术后视力情况。术后随访3个月。结果术后随访裸眼或矫正视力≥0.5者35只眼(95%),术中上方玻璃体脱出1只眼(2.7%),切口周围角膜轻度水肿6只眼(16%),无其他重大并发症发生。结论临床上极硬核白内障的治疗是棘手问题,术中处理不当会出现后囊破裂、玻璃体脱出、角膜水肿及失代偿等严重并发症发生。采用小切口非超声乳化白内障摘除术,操作相对简单,手术时间和手术效果与超声乳化手术效果基本一致,避免了严重并发症的发生,取得良好效果。OBJECTIVE To observe the clinical value of small incision phacoemulsification in treatment of cataract with a hard core.METHODS 31 cases with 37 eyes with very hard nuclear cataract accepted a small incision cataract through the phacoemulsification treatment,and we recorded the operation process,observation records operative complications and visual acuity.The follow-up period was 3 months.RESULTS 35 eyes were followed up,and the visual acuity were corrected to 0.5（95%）,one eye showed vitreous emerge during the surgery（2.7%）.6 eyes showed mild edema in the corneal incision（16%）,and no major complication occurred.CONCLUSION Treatment for hard nuclear cataract is a tough questions.There is a risk of later capsule rupture during surgery,andvitreous emerge,corneal edema and decompensate severe complication.Small incision cataract with phacoemulsification is a relative simple operation.The operation time,operation effect and operation effect of phacoemulsification are basic agreement,and we should avoid the serious complications,and achieve good effect.
