高血压病并发心房颤动血栓前状态标志物与左心室舒张功能参数的关系研究

摘　　要：目的探讨高血压病并发心房颤动患者血栓前状态标志物水平与左心室舒张功能参数[二尖瓣舒张早期峰值血流速度与二尖瓣环舒张早期峰值运动速度均值之比（E／Em）]的相关性。方法随机选取健康体检者（A组）50例，单纯高血压病患者（B组）60例，高血压病并心房颤动患者（C组）64例，用免疫比浊法测定各组血浆高敏C．反应蛋白（hs-CRP）、D．二聚体（D．dimer）浓度，采用酶联免疫吸附法（ELISA）法测定血浆血管性假血友病因子（vWF）浓度，并用多普勒超声心动图测定各组左心室舒张功能参数E／Era值，分别比较各组hs-CRP、D．dimer、vWF与E／Era值以及其间的相关关系。结果C组血栓前状态标志物（hs—CRP、D-dimer、vWF）水平与E／Em值均高于A、B组，差异有统计学意义（P〈0．05）。相关分析显示：高血压病并心房颤动患者血浆hs．CRP、D-dimer、vWF水平与E／Em值呈正相关性（r值分别为0．59，O．64，0．67，P〈0．05）。结论与单纯高血压病组患者相比，高血压病并心房颤动组患者血栓前状态标志物水平与左心室舒张功能参数E／Em值明显升高，两者正相关性更强。Objective To investigate the correlation of plasma prethrombotic state marker with parameter of left ventricular diastolic function [ the ratio of mitral inflow velocity and mitral annular velocity in early period（E/Em） 1 in patients with hypertensive disease complrcated with atrial fibrillation. Methods Totally 50 cases in physical examination（group A）, 60 patients with hypertensive disease（ group B）, and 64 patients with hypertensive disease complicated with atrial fibrillation（group C） were randomly selected. The concentrations of plasma high sensitivity C reactive proein （hs-CRP） and D-dimer （D-D） were deter- mined by immunoturbidimetry. And concentrations of plasma yon Willebrand factor（vWF） were determined by enzyme-linked immunosorbent assay（ELISA）. The parameters of left ventricular diastolic function （E/ Era） were measured by Doppler echocardiograph. The levels of prothrombotic state marker（hs-CRP, D-di- mer,vWF） ,the ratio of E/Em and correlativity were compared. Results The levels of plasma hs-CRP, D- dimer, vWF and the ratio of E/Em in group C were significanly higher than those in group A and B （P 〈 0. O1 ）. Compared with group A and B, there was positive correlation of prothrombotic state marker with E/oEm in group C （P 〈 0. 01 ）. Conclusions The concentrations of plasma prethrombotic state marker, parameter of left ventricular diastolic function（E/Em） and the correlation were significanly higher in patients with hyper- tensive disease combined with atrial fibrillation than those of hypertensive disease.
