摘  要:目的应用三维应变率成像研究收缩期应变率峰值及收缩后收缩应变率峰值对冠心病的早期诊断价值。方法对46例心肌缺血患者和48例健康者进行三维应变率成像检查,记录三维四腔动态应变率图像,脱机后利用随机所带QTVI定量分析软件进行应变率曲线分析,并做统计学处理。结果正常组收缩期应变率曲线呈“V”形,节律规整,各节段间协调性好,收缩后收缩现象少,峰值低,占时短;缺血组应变率曲线呈锯齿状、“W”形、“r”形或杂乱形,收缩期应变率峰值减低,收缩后收缩现象普遍存在,而且收缩后收缩应变率峰值高,占时长。结论三维应变率成像检查耗时短,费用低,收缩后收缩应变率对冠心病心肌缺血的敏感性高,可作为冠心病筛查的一种常规手段使用。Objective Application of three-dimensional SRI to study the tolic and PSS on the early diagnosis value of coronary heart disease. Methods strain rate peak of sys- F.oryt-six cases of myo- cardial ischemia and 48 cases of normal were examined by three-dimensional SRI. Three-dimensional dy- namic SRI of four-chamber view was recorded. The random onboard QTVI quantitative analysis software was used to analysis strain rate curve and statistical analysis was done. Results In normal group, systol- ic strain rate curve was "V"-shaped, regular rhythm, well coordination inter the various segments, less post systolic shortening, low peak and short holding time. In ischemic group, strain rate curve jagged were "W" shape, "r" shaped and messy shape. The peak of systolic strain rate reduced. The post sys- tolic shortening was normal. The SRI peak of PSS was high and long holding time. Conclusions The three-dimensional SRI examination is time-consuming short, low-cost. PSS strain rate is highly sensitive on coronary heart of myocardial ischemia. It can be used as a routine means of coronary heart disease screening.
