摘　　要：目的了解昆山市乡镇医院的医院感染控制现状,以便制订医院感染管理措施,为创建小康型乡镇卫生院提供依据。方法通过自制调查表、访谈交流及现场查看等方法对9所乡镇医院,1所社区服务中心在医院感染综合管理及消毒隔离等方面进行调查。结果有医院感染管理三级网络的医院6所,占60%;开展灭菌物品和环境卫生学监测的医院10所,占100%;开展手卫生和医院感染病例监测的医院占80%;开展病原微生物学监测的医院有2所,占20%;医疗废弃物管理的合格率为100%;污水处理合格率为30%;紫外线灯有强度监测的医院占90%;有专用内镜清洗设备的医院占100%,用戊二醛消毒内镜的医院占77.8%,用酸水消毒内镜的医院占22.2%;医院感染知识培训达标率和一次性物品使用合格率为100%;抗菌药物合理使用上,经验用药占100%。结论昆山地区乡镇医院的医院感染管理体系较为健全,但仍需要加强管理队伍建设,提高感控人员的素质;增加资金投入,更新消毒设备,添置一些必要的手卫生设施和设备,加强污水管理,减少医源性感染的发生;进一步加强消毒隔离制度的落实和医院感染知识的宣传与培训力度;加强综合管理,有效促进抗菌药物的合理使用。Objective To understand the current hospital infection control status of Kunshan township hospitals, so as to regulate hospital infection control measures and provide the basis for creating the new well-off society. Methods Hospitals＇ infection comprehensive management, disinfection and isolation and other aspects were investigated by the means of a self- produced questionnaire, interviews, on-site investigation and other methods. 9 township hospitals and I community service center were included in this investigation. Results The level 3 network hospital infection management was possessed by 6 Kunshan township hospitals, accounted for 60%; the sterilization of items and environmental sanitation were monitored by 10 hospitals, accounted for 100%; the hand hygiene and hospital infection cases monitoring were brought into use in 80% of the hospitals; microbiological monitoring was held by 2 hospitals, accounted for 20%; The pass rate of medical wastes management was 100%; the pass rate of sewage treatment was 30%; ultraviolet lamps were equipped with intensity monitor in 90% of the hospitals; 10 hospitals had special endoscopic cleaning equipment, accounted for 100%, 77.8% used glutaraldehyde to sterilize the endoscope and 22.2% used acid water; the pass rate for hospital infection knowledge training and disposable amenities usage were 100%; 100% of the hospitals were found using medication experience in the application of antimicrobial agents. Conclusion Although the hospital infection control department in Kunshan township hospitals is wholesome, there is still need to develop a more comprehensive and professional management team and improve the qualities of its members. Furthermore, hospitals should increase the fund to update the sterilize equipment, add necessary hand hygiene facilities, strengthen sewage treatment and reduce iatrogenic infection.
