游离皮瓣移植不同血管吻合方式比较分析

摘　　要：目的 比较不同血管吻合方式对游离皮瓣移植成活的影响.方法 根据血管吻合方式不同,将游离皮瓣移植患者分为三组,A组采用手术放大镜下缝线吻合法,B组采用显微镜下缝线吻合法,C组采用血管吻合器吻合.比较三种血管吻合方式的血管吻合时间、游离皮瓣缺血再灌注时间及术后并发症.结果 采用血管吻合器吻合时间及缺血再灌注时间明显短于显微镜下缝线吻合和放大镜下缝线吻合,后二种方法差异无统计学意义.56例移植游离皮瓣中,6例患者发生血管危象（放大镜下缝线吻合组和显微镜下缝线吻合组各3例,血管吻合器吻合组无一例发生血管危象）,经积极处理后全部成活,移植成功率100%.结论 游离皮瓣移植应用血管吻合器吻合可以简化手术操作,缩短血管吻合时间,进而减少皮瓣缺血再灌注时间,提高移植成活率.Objective To study and compare the effect of different vascular anastomosis methods for free flaps transplantation. Methods According to the methods of vascular anastomosis the subjects were divided into three groups： suture method under microscope （ group A ） , suture method with loupe （ group B ） , vascular anastomosis device （ group C ） . The ischemia time of the flaps, anastomosing time and postoperative complications were observed and compared between these three groups. Results The ischemia time of the flaps and the anastomosing time in group A and B were significantly less than that in group C. There were 6 complications among these 56 flaps, all the flaps survived after appropriate management, the successful rate of anastomosis was 100%. Conclusions Anastomosis device can simplify the surgical procedure, significantly shorten the anastomosis time and ischemia time of the flaps, then increase the survival rate of the free flaps transplantation.
