地塞米松预防新生大鼠缺氧缺血性脑病的作用

摘  要:目的研究地塞米松(Dex)预处理对新生大鼠缺氧缺血性脑病(HIE)的预防作用及其机理。方法 4 d龄SD大鼠随机分为 3组,分别给予生理盐水[10 ml(kg·d)]、低剂量 Dex[0.1mg/(kg·d)]、高剂量 Dex[0.5 mg/(kg·d)]预处理4 d,然后结扎左侧颈总动脉,放入 37℃恒温有机玻璃缺氧房中,暴露于 92%氮气和 8%氧气的环境中 3 h。分别于生后10 d取全脑标本切片作 HE染色,观察梗死发生率、梗死面积、大脑皮层细胞超微结构变化。结果低剂量 Dex组、高剂量Dex组的梗死发生率和梗死面积均低于生理盐水组(P< 0.001),Dex组间差异无显著性( P> 0.05)。 Dex可明显改善大脑神经细胞的超微病理学变化。结论地塞米松预处理对新生大鼠缺氧缺血性脑损害具有预防作用,其神经保护机制可能是通过延缓神经发育成熟实现的。Objective To investigate the effects of dexamethasone pretreatment on neonatal HIE. Method All 4 - d - old Sprague - Dawley rat pups (n = 60) were randomized to one of three treatment groups and beated with either 0. 1 mg/kg saline or 0. 1 or 0.5 mg/kg dexamethasone for 4 consecutive days. On the 7th postnatal day, arter ligation of the left common carotid artery, the rats were exposed to 8% oxygen - 92% nitrogen at 37℃ for 3 hours and decapitated on the 10th postnatal day. The baains were subsequently fixed by perfusion and stained with hematoxylin - eosin. The area and incidence of infaretion, the supermicro - construction of neuronal cells in cortex were observed. Rasults The infarction was smaller and the area incidence of infaretion were lower in both Dex groups than in con- trols (P < 0.00l). The rats in both Dex groups showed milder damage of the supermicro - construction of neuronal cells in cortex. Con- clusion Dexamethasone pretreatment prevents HIE through suppression of neuronal maturation in neonatal rat.
