椎弓根螺钉联合连接棒内固定法治疗胸腰段脊柱骨折的临床疗效

摘　　要：目的评价椎弓根螺钉联合连接棒内固定治疗胸腰段脊柱骨折的临床疗效。方法选择32例胸腰段脊柱骨折患者为研究对象,采用椎弓根螺钉联合连接棒内固定技术进行治疗。评价Cobb角、椎体前缘高度、椎体后缘高度和脊髓损伤Frankel分级,观察术前、术后及随访2年后的变化。结果手术后32例患者的Cobb角(4.8±0.2)与术前(25.3±0.9)相比差异具有统计学意义(P<0.05),椎体前缘高度(32%)和椎体后缘高度术前(73%)和术后相比差异均具有统计学意义(P<0.05),分别为93%和96%,并且这三项在随访2年后均无显著变化。神经功能方面,除2例A级外,其他患者神经功能均有1～2级恢复。结论椎弓根螺钉联合连接棒内固定治疗胸腰段脊柱骨折临床疗效好,无不良反应发生,也无断钉、钉松现象,具有较高的稳定性。Objective To evaluate the clinical effect of thoracolumbar fractures treated with pedicle screw and rods internal fixation. Methods Thirty-two patients with thoraclumbar fractures were selected in our hospital. The treatment method was pedicle screw fixation and rods. The Cobb angle, anterior vertebral height and posterior edge height were evaluated pre-operative and postoperative. Results The Cobb angle was （25.3±0.9） atpreoperative, while the value dropped to 4.8±0.2 at postoperative. The anterior vertebral height was changed from 32% to 93% and the posterior edge height was from 73% to 96%. There were statistical significance among the three indexes （P〈0.05）. we are so glad that the values have not changed greatly after follow-up of two years. The neurologic function of all 32 patients was improved by at least one to two grades, except two patients with A grade. Conclusion Results showed that thoraeolumbar fractures treated with pedicle screw fixation and rods was effective without side effects. And the combination method could provide reliable neurologic improvement in patients with incomplete neruolgic deficit.
