摘　　要：社区卫生服务是我国当前卫生事业的重要内容。本研究在探索健康管理和基本医疗的"分离模式"的基础上,在实践应用中不断丰富完善,建立了"双轨互促"式社区健康管理模式,并对该模式的服务流程及岗位进行详细描述及分析论证,以供社区卫生服务工作者借鉴参考。Community health service is a very important part of public health in China.Based on the ＂separation mode＂ of health management and basic medical care,we have continuously improved the mode and eventually establish the ＂Dual Mutual Promotion Mode＂ of community health management.Here we describe and analyze the service process and post of this mode,so as to provide a reference for urban community health workers.
