摘 要:目的研究硝酸甘油和甲基强的松龙预处理供体减轻肺移植后因热缺血加重的缺血-再灌注损伤的作用。方法利用兔肺离体再灌注模型。实验共分4组,合并热缺血15 min后都用改良的4℃MEC液灌洗,保存7 h后进行再灌注。WI为单纯有热缺血的对照组,MP、NC和M+N组为用药组,分别用甲基强的松龙、硝酸甘油及甲基强的松龙联合硝酸甘油预处理供体1 h,再灌注持续30 min,用这时的血氧差、平均肺动脉压、气道峰压和湿干重比来评价供肺功能。结果 MP和M+N组的肺功能较WI组和NC组有明显改善。结论在本实验模型中甲基强的松龙能改善热缺血加重的缺血-再灌注损伤,硝酸甘油未见相同作用。Objective To study the effect of Nitrogly cerinurn and Methelpredisolone in pretreatment of donor lung to alleviate aggravated ischemia-reperfusion injury induced by warm ischemia after lung transplantation. Methods Rabbits were used for experiment and divided into four groups. After fifteen minutes warm-ischemia, the donor's lungs were flushed with 4℃ MEC solution and stored 7 hours before reperfusion. WI group was as the control group with single warm ischemia. MP, NC and M + N groups were as the treatment group, whose donor's lungs were respectively by pretreatment of Methelpredisolone, Nitrogly cerinum and Methelpredisolone combined with Nitrogly cerinmn, and then were flushed for 30 minites. Lung function was evaluated by difference of PaO2 ( △PaO2 ), medial pressure of pulmonary artery (mPAP) , peak airway pressure (pAP) , and ratio of wet/dry weight (W/D). Results The lung function of MP and M + N group was obviously improved than that of W1 group and NC group. Conclusion Methelpredisolone pretreatmcnt can attenuate aggravated ischemia-reperfusion injury, while Nitrogly cerinum showed no such effect in this model.
