208例呼吸科住院死亡患者临床分析

摘　　要：目的对我院3年住院死亡病例进行临床分析。方法收集住院患者中所有死亡患者208例的临床资料,并进行分析。结果死亡患者占同期住院患者6.2%,收入ICU患者占61.5%,平均住ICU为17.3天(1～153 d),无创97人,有创85人,其中先无创,后有创62人。单纯无创35人,单纯有创23人。死亡患者以男性为多。死亡患者年龄平均71岁(21～94岁),60岁以上占81.2%。死亡患者有10种以上疾病者占49%,4种疾病以下者仅占7.7%。患者死亡原因前3位为肺炎、肿瘤和多脏器功能衰竭,占总人数的55.8%。其中呼吸道疾病占58.7%,非感染性疾病占55.3%。结论呼吸科住院死亡患者男性多于女性,入ICU救治行呼吸机辅助通气比例高,高龄、有多种疾病并存是死亡的主要高危因素。Objective To analyze the clinical characteristics of the eases of death hospitalized in the past three years in order to improve the doctor＇s level of diagnosis and treatment. Methods clinical data of 208 cases from 2008 - 3 to 2011 - 3 were collected and analyzed. Results The patients who died accounted for 6.2% of hospitalized patients during the same period, ICU patients accounted for 61.5% , the average ICU stay was 17.3 days （ 1 - 153 days）, the patients with non-invasive ventilation （NIV） were 97 cases, invasive ventilation （IV） were 85 cases, endotracheal intubation after NIV failure were 62 cases, just NIV were 35 cases, and IV were 23 cases. Death in patients, male were more than female, and ratio of male： female was 1.6 ： 1. Death in patients with a mean age of 71 years （21 -94 years）, and patients more than 60 years accounted for 81.2% 49%, and less than four kinds of diseases only accounted for 7.7% The patients who died more than 10 kinds of diseases accounted for The death reasons of first to three were pneumonia, cancer and mul- tiple organ failure, and they accounted for 55.8% of all the death. Respiratory diseases accounted for 58.7%, and non-infectious diseases accounted for 55.3%. Conclusion In the death hospitalized in respiratory wards, male are more than female, a high proportion of admission to the ICU for treatment and more patients need mechanical ventilation. The major risk factors for death are senior citizens with eoxist more kinds of diseases.
