摘 要:目的探讨三维超声彩色能量成像(3D-CPA)联合彩色多普勒超声(CDFI)技术定量评价妊娠高血压综合征(PIH)患者胎盘血流情况的价值。方法 90例PIH孕妇中,轻度子痫前期53例(P-L组),重度子痫前期37例(P-H组),同期选取100例正常孕妇为正常对照组(N组)。对所有孕妇胎盘床血管进行三维重建,采用VOCAL软件测量血管化指数(VI),血流指数(FI),血管化血流指数(VFI)。采用CDFI测量脐动脉收缩期/舒张期血流速度峰值(S/D),并与正常对照组比较。结果 P-L组、P-H组与N组S/D值比较,差异有统计学意义(P<0.01),P-H组高于P-L组、N组(P<0.01)。P-L组、P-H组VI、FI、VFI与N组比较,差异有统计学意义(P均<0.01),均为P-H组<P-L组<N组。结论 PIH患者S/D值升高,VI、FI、VFI值降低,且以重度子痫前期为明显。3D-CPA技术可更早更精确地提示PIH患者的胎盘血流灌注情况。Objective To investigate the clinical value of three-dimensional color power angiography(3D-CPA) and color Doppler flow imaging ( CDFI ) in the quantitate assessment of placental perfusion in pregnancy-induced hypertension(PIH) patients. Methods Ninty pregnant women with PIH were divided into the mild pre-eclampsia group( n = 53, Group P-L) and severe pre-eclampsia group (n = 37, Group P-H), 100 healthy pregnant women as the controls( Group N). Three-dimensional reconstruction of placental vascular was performed,and placental vascularization index(V I),flow index(F[),and vascularization-flow index(VFI) were measured with VOCAL software. CDFI was used to measure the umbilical blood flow systolic to diastolic (S/D). The data in PIH patients were compared with the controls. Results There was significant difference in S/D between Group P-L and Group N, as well as between Group P-H and Group N ( P 〈 0.01 ), and S/D was significantly higher in Group P-H than in Group P-L, and Group N ( P 〈0.01 ). VI,FI and VFI showed significant difference between Group P-L and Group N,as well as between Group P-H and Group N(all P 〈0.01) ,which were lowest in Group P-H and highest in Group N. Condusion S/D increased and VI, FI,VFI decreased in PIH patients,especially in the patients with severe pre-eclampsia. 3D-CPA could suggest placental perfusion in PIH patients earlier and more accurately.
