摘  要:目的观察多发伤所致ALI患者BNP水平的变化,评价BNP对ALI、严重程度及预后评估的价值,同时评估BNP对急性肺损伤中机械通气患者撤机结果的预测价值。方法通过测定诊断为ALI的患者60例(ALI组)及健康体检者58例(对照组)的血浆BNP浓度,比较两组BNP的水平;根据肺损伤评分,研究ALI组患者肺损伤的严重程度与血浆BNP浓度变化的关系;因病情严重需要机械通气患者,依撤机结果分为撤机成功组和失败组,比较两组间撤机前BNP的差异。结果 ALI组血浆BNP水平较对照组显著升高(P<0.01),且与ALI严重程度密切相关(P<0.01);撤机失败组BNP水平明显高于成功组(P<0.01)。结论BNP水平可成为ALI,评价ALI严重程度及机械通气撤机的重要标志物。Objective To observe B - type natriuretic peptide (BNP) levels in acute lung injury (ALI) patients resulting from mul- tiple injuries. To explore the value of BNP in diagnosing, assessing the severity of ALI, and predicting the weaning outcome of mechanical ventilation. Methods The plasma concentration of BNP was measured in both ALl group ( n = 60) and healthy control group ( n = 58 ) . BNP levels were compared between the two groups, also among different degrees in ALI group. Meanwhile, according to weaning outcome of mechanical ventilation, BNP levels were compared between the success group and the failure group. Results Compared to the control group, BNP level in ALI group significantly increased (P 〈 0.01 ), which was closely related to the severity of pathogenetic condition (P 〈 0.01 ) ; additionally, BNP level was obviously higher in the failure group than that in the success group ( P 〈 0.01 ). Conclusion BNP may be an ideal index in the diagnosis and evaluating severity of ALI, also an important marker to predict the weaning outcome of mechanical ventilation.
