子宫内膜异位症患者盆腔疼痛与病灶特点的相关性分析

摘　　要：目的探讨子宫内膜异位症(内异症)患者盆腔疼痛与病变特点的关系。方法选择2007年2月至2010年12月经腹腔镜及病理检查确诊的子宫内膜异位症患者153例,回顾性分析其盆腔疼痛与病灶特点的关系。结果 153例内异症患者中,22例单纯子宫内膜异位囊肿者无盆腔痛,131例子宫内膜异位囊肿合并腹膜病灶、盆腔粘连及深部浸润型结节者均有不同程度的盆腔痛。在盆腔痛的发生率上,48例病变位于子宫骶韧带及53例病变位于子宫直肠凹的患者盆腔痛发生率明显高于其它部位的病变,而且83例中重度盆腔粘连者、85例深部浸润型患者盆腔疼痛发生率均明显高于38例轻度盆腔粘连者、46例浅表型患者(P﹤0.05)。结论子宫内膜异位症盆腔疼痛与病灶特点关系密切,其中盆腔粘连、子宫直肠凹及骶韧带的深部病灶是盆腔疼痛的重要因素。Objective To investigate the relation between pelvic pain and lesion features in patients with endometriosis. Methods 153 patients with endometriosis diagnosed by iaparoscopy and pathology from Feb 2007 to Dee 2010 were analyzed retrospectively and the re- lationship between pelvic pain and lesion features was evaluated. Results In 153 patients with endometriosis, the pelvic pain was not found in 22 patients with simple endometrial cyst and 131 patients with simple endometrial cyst complicated peritoneal lesion, pelvic adhesion and deeply infiltrating nodules suffered from chronic pelvic pain on the contrary. The incidence rates of chronic pelvic pain in 48 cases with infil- trating lesion in uterosacral ligaments and 53 cases with deeply infiltrating nodules in reetouterine pouch were significantly higher than those in other cases. Moreover, the incidence rates of pelvic pain in 83 cases with moderate or serious pelvic adhesion and in 85 cases with deeply infihrating lesions were remarkably higher than those in 38 cases with mild pelvic adhesion and 46 cases with superficial infiltrating lesions, respectively （P 〈 0.05 ）. Conclusion The pelvic pain of endometriosis is notably related to endometriotic lesion features, and pelvic adhe- sion and deeply infiltrating lesions in uterosacral ligaments and rectouterine pouch are the most frequent reasons resulting in pelvic pain.
