氨溴索和多索茶碱联合治疗COPD稳定期患者的疗效分析

摘　　要：目的探讨氨溴索和多索茶碱联合治疗中重度慢性阻塞性肺疾病(COPD)稳定期患者的临床疗效。方法将90例患者随机分为三组,每组30例,A组给予氨溴索片30mg/次,3次/天;B组给予多索茶碱片,0.2g/次,2次/天;C组给予氨溴索片和多索茶碱片,其中氨溴索片30mg/次,3次/天,多索茶碱片,0.2g/次,2次/天;疗程均为12个月。治疗前后测定肺功能、圣.乔治呼吸问卷评分(SGRQ)、6min步行距离(6MWD)和急性加重期发作次数。结果氨溴索和多索茶碱联合治疗组较其他两组的肺功能指标FEV1、FEV1占预计值百分比和6min步行距离有明显改善,圣·乔治呼吸问卷评分和急性加重期发作次数显著减少,具有统计学意义。结论氨溴索和多索茶碱联合能有效改善COPD稳定期患者的肺功能和生活质量、提高运动耐力及减少急性加重期发作次数,其疗效明显优于单用氨溴索或多索茶碱,且用药安全性好,值得推广。Objective To investigate the clinical effect of ambroxol plus doxofylline in the treatment of patients with stable chronic obstructive pulmonary diseas （COPD）. Methods 90 patients with COPD were randomly divided into three groups, with 30 patients in each group. The group A was given ambroxol tablets, 30 mg each time, 3 times one day; the group B was given doxofylline tablets, 0.2 g each time, 2 times one day; the group C was given ambroxol tablets, 30mg each time, 3 times one day and doxofylline tablets 0.2 g each time, 2 times one day; all for 12 months. The lung function, St George＇s respiratory disease questionnaire （SGRQ）, 6 min walking distance （6MWD） and the times of acute exacerbation were measured before and after treatment. Results Ambroxol plus doxofylline group was more than the other two groups in improving the lung function FEV1, FEV1% and 6 min walking distance, and reducing the scores of SGRQ and the times of acute exacerbation, with statistically significant difference. Conclusions Ambroxol plus doxofylline can significantly improve the lung fimction, quality of life and exercise capacity in patients with COPD, and reduce the times of acute exacerbation, with obviously better curative effects than those of ambroxol or doxofylline used alone, which are safe and worthy of application.
