摘　　要：目的:通过临床药师参与临床药物治疗,对患者进行药学监护,促进合理用药。方法:以一例风湿性心脏病行二尖瓣置换术合并癫痫患者为例,临床药师参与药物治疗方案的制订,分析患者所用相关药物,跟踪其治疗效果及不良反应,对药物治疗过程进行综合评估。结果:患者症状控制,治愈出院。结论:临床药师的参与使药物治疗更趋合理,在达到治疗目标的同时,可减少药物的不良反应,有利于提高临床的药物治疗水平。Objective：Clinical pharmacists involved in clinical drug therapy and pharmaceutical care for patients to promote rational drug use in clinics.Method：In the whole therapy process Clinical pharmacists communicated with the clinician to design therapeutic scheme,analyses medications and evaluate their effectiveness and adverse reaction for a patient with rheumatic heart disease and atrial fibrillation.undergoing valve replacement and epilepsy.Result：A1l the indices of the patient trended toward norma1 and the patient were recovered from his disease Conclusion：The participation of clinical pharmacists in clinical treatment team and clinical practice can make drug treatment more rational with therapeutic goal reached and adverse drug reactions reduced to improve the level of clinical drug treatment.
