老年高血压患者动态脉压(PP)与冠心病并发症及颈动脉粥样硬化的关系研究

摘　　要：目的探讨老年高血压患者的动态脉压(PP)与冠心病并发症及颈动脉粥样硬化之间的关系。方法选取2006年12月～2012年1月间于某院门诊或住院治疗的老年高血压患者341例,其中合并冠心病的165例病例为A组,单纯的老年高血压患者176例为B组,监控两组动态脉压,对比观察两组动态PP的差异;另对B组176例病例根据颈动脉内膜中层厚度(IMT),IMT≥0.9 mm为颈动脉内膜中层增厚组,共91例为α组,其余85例为β组,比较两组动态PP的差异。结果合并冠心病组和颈动脉内膜中层增厚组的24 h APP、dPP、nPP水平明显高于单纯高血压组和颈动脉内膜中层厚度正常组。结论动态脉压水平对冠心病并发症和颈动脉粥样硬化病变有提示作用,动态脉压越高,冠心病并发症和颈动脉粥样硬化病变发病率越高。OBJECTIVE To investigate the relationship of elderly hypertensive patients with ambulatory pulse pressure （PP） and coronary heart disease complications and carotid atherosclerosis. METHODS 341 cases of elderly hypertensive patients in the hospital outpatient or inpatient treatment from Deeember 2006 to January 2012 were chosen as the research objects. 165 cas- es of CHD were set as group A, the mere 176 cases of elderly patients with hypertension were set as the B group. The ambula- tory pulse pressure of the two groups was monitored. The differences of dynamics of PP for the two groups were observed. Another 176 cases in group B according to carotid intima-media thiekness （IMT）, IMT≥ 0.9 mm for the carotid intima-media thick- ening of the group, 91 cases were set as the ot group, the remaining 85 eases were set as 13 group. The differenees of the dy- namic PP were compared. RESULTS The levels of 24hAPP, dPP, nPP level for group with coronary heart disease group and carotid intima-media thickening were significantly higher than the normal group of simple hypertension and carotid artery intima- media thickness. CONCLUSION The level of ambulatory pulse pressure （PP） has effect for coronary heart disease complica- tions and carotid atheroselerosis. The higher ambulatory pulse pressure, the higher the incidence of coronary heart disease com- plications and carotid atherosclerotic lesions.
