摘 要:目的了解河南地区产超广谱β-内酰胺酶(ESBLs)大肠埃希菌的流行情况,分析产ESBLs菌的耐药性。方法全省各地共17家医院采取统一方法、内外部质量控制,进行临床分离菌的检测,然后应用WHONET5.5、SPSS11.0软件对监测资料进行汇总、分析。结果 5020株大肠埃希菌中共检出产ESBLs菌3694株,检出率为73.6%。在各类标本中,尿中产ESBLs菌株分离率最高(40.7%),其次是痰(21.9%)。除美罗培南、头孢哌酮/舒巴坦、头孢西丁外,产ESBLs菌株与非产ESBLs菌株相比,对其他16种抗菌药物的耐药率差异有统计学意义(P<0.05)。结论河南地区产ESBLs大肠埃希菌株对多类药物表现出较高的耐药率。Objective To study the epidemic situation of Escherichia coil producing extended-spectrum β-lactamases(ES- BLs) in Henan district and to analyze its drug resistance. Methods Clinical isolates were detected applying a unified approach, intemal and external quality control in 17 hospitals across the province, then, the data were collected and analyzed by WHONET5.5 and SPSS 11.0 software. Results A total of 3 694 strains of ESBLs were isolated from the 5 020 strains of Eseherichia toll, the isolation rate was 73.6%; the highest isolation rate of strains of ESBLs was gained from the specimen of urine (40.7%) and then was from the specimen of sputum (21.9%); except meropenem, cefoperazone and sulbactum, cefoxitin, ESBLs-producing strains and non-ESBLs producing strains, the other 16 kinds of antimicrobial resistance rates a significant difference (P 〈 0.05), had a much stronger drug resistance against other 16 antibiotics than non-ESBLs-produc ing strains (P 〈 0.05). Conclusion The ESBLs-produeing Escheriehia coli strains have a stronger drug resistance to many kinds of antibiotics in Henan district.
