摘 要:目的:了解上海社区中老年人超重和肥胖的患病情况以及肥胖相关疾病的患病风险。方法:在上海市嘉定区40岁及以上居民中进行问卷调查、体格检查及生化检测.对其中数据完整的10375名居民进行统计分析。根据体质量指数(BMI)。按照WH0标准界定肥胖和超重者。采用线性回归法分析BMI与一些危险因素的相关性,并采用Logistic回归法分析超重以及肥胖状态对于各种代谢相关性疾病患病风险的影响。结果:本研究人群的BMI均值为25.1±3.3.超重和肥胖的患病率分别为42.92%和7.27%;多元线性回归分析显示:校正多种混杂因素后,随着BMI水平的上升.腰围、收缩压、舒张压、空腹血糖、餐后2h血糖、总胆固醇、低密度脂蛋白胆固醇、三酰甘油随之增加(均P〈0.01).高密度脂蛋白胆固醇随之下降(P〈0.0001);多元Logistic回归分析显示,超重和肥胖会增加高血压、糖尿病、血脂异常、代谢综合征以及卒中患病风险[比值比(OR):1.50。13.2,均P〈O.0001)1。结论:上海嘉定社区中老年人群超重和肥胖的患病率分别为42.92%和7.27%。超重和肥胖是高血压、糖尿病、血脂异常、代谢综合征以及卒中等肥胖相关疾病的风险因素。Objective To study the prevalences of overweight and obesity in middle-aged and elderly people of a community in Jiading district, Shanghai, and to evaluate the correlation between overweight/obesity status and obesity- related diseases. Methods Ten thousand three hundred and seventy-five subjects aged 40 years or older from Jiading district, Shanghai were recruited to undergo questionnaire interview, anthropometric measurement and biochemical evaluation. There were 3 957 men and 6 418 women. In accordance with the World Health Organization (WHO) criteria, overweight and obesity were defined as a body mass index(BMI) equal to or greater than 25 and 30 , respectively. Linear regression model was used to analyze the relation between BMI and other related risk factors, and Logistic regression analysis was used to analyze the effect of overweight/obesity on prevalence of metabolic-related diseases. Results In this study, the mean BMI was 25.1+3.3 and the prevalences of overweight and obesity were 42.92% and 7.27%, respectively. Multiple linear regression analysis revealed that with the increase of BMI, waist circumference, systolic blood pressure (BP), diastolic BP, fasting plasma glucose (FPG), postprandial 2 h plasma glucose (2hPG), total cholesterol (TC), low density lipoprotein cholesterol (LDL-C), triglyceride (TG) increased (all P〈0.01) and high density lipoprotein cholesterol (HDL-C) decreased (/3=-0.027, P〈0.000 1). Logistic regression analysis demonstrated that both overweight and obesity were the independent risk factors for hypertension, diabetes, dyslipidemia, metabolic syndrome and stroke [odds ratio (OR) from 1.50 to 13.2, all P〈0.000 1]. Conclusions The prevalences of overweight and obesity in middle age and elderly people were 42.92% and 7.27%, respectively. Both overweight and obesity were independent risk factors for obesity- related disease such as hypertension, diabetes, dyslipidemia, metabolic syndrome and stroke.
