64排128层CT冠脉成像对心肌桥—壁冠状动脉的诊断价值

摘　　要：目的探讨64排128层CT冠状动脉血管造影(CTA)对心肌桥-壁冠状动脉的诊断价值。方法采用64排128层CT对212例有冠心病症状或心脏体检患者行冠状脉成像检查,分析心肌桥-壁冠状动脉的发生率、发生部位、长度、厚度、狭窄程度及心肌桥近端管壁粥样硬化情况。结果 212例中CTA发现心肌桥55例(64处心肌桥),阳性率25.9%;46例为单一心肌桥,9例为两处受累,同时累及前降支近段、钝缘支5例,同时累及前降支中段、后降支4例;其中前降支48处,第一对角支3处,第二对角支1处,左旋支1处,钝缘支5处,右冠状动脉4处,后降支2处,以前降支最多见,占75%。左前降支心肌桥组近端较无心肌桥组近端冠状动脉粥样硬化发生率高。结论 64排128层CT冠状动脉血管造影可准确诊断心肌桥—壁冠状动脉,为临床诊断和治疗提供有力的依据。Objective To evaluate the application value of the 64 rows -128 slices CT coronary angiography in diagnosing myocardial bridge-mural coronary artery （MB-MCA）. Metho.rd8 64 rows -128 slices CT coronary angiography was undergone in 212 patients who had Symptom of coronary artery disease or no any symptoms. The prevalence, location, length, depth, narrowing degree, proximal atherosclerosis of MB- MCA were analyzed. Results Of the 212 patients, 55 （25. 9%）patients with a total of 64 coronary segments were found to have MB-MCA. 46 cases had one MB-MCA, 9 cases had two involved, which were located in the proximal segment of the left anterior descending artery （LAD）and obtuse marginal branch, the mid segment of LAD and posterior descending artery（PDA）, respectively.LAD： 48 segments, first diagonal branch （FD）：3segments, second diagonal branch（SD）： lsegments, left circumflex artery（LCX）：lsegments,left marginal branch （LMB）： 5segments, right coronary artery（RCA）： 4segments, posterior descending artery（PDA）： 2segments, the LAD was the most common coronary artery involved （75%,48/64）. the prevalence of Proximal Atherosclerosis of LAI） myocardial bridge group was higher than tile control group＇ s. Conclusion the 64 rows-128 slices CT coronary imaging carl diagnose MB-MCA accurately and provide a strong basis for clinical diagnosis and treatment.
