摘 要:目的探讨MRI多技术扫描评价心肌病变的临床应用价值。方法收集MRI检查资料完整者23例,均进行MRI平扫、电影扫描、心肌灌注和延迟增强扫描,观察心脏形态、运动、首过灌注、延迟强化信号改变,分析其诊断价值。所有检查者均经临床确诊。结果 23例中,12例为缺血性心肌病,共有204心肌节段被分析,63段(30.9%)见异常室壁运动;异常运动节段中,首过灌注心肌信号减低52段,信号正常11段,59段(93.7%)有延迟强化,所有运动异常节段室壁增厚率均下降。2例肥厚型心肌病、1例扩张型心肌病、1例心脏肿瘤病灶均见延迟强化。结论 MRI多技术扫描可较准确评价心肌病变,具有重要的临床应用价值。Objective To evaluate the clinic application value ofmultimodality MR imaging in assessment of myocardial lesion. Methods 23 cases underwent multimodality MR imaging, including morphology scanning, cine MRI, myocardial perfusion and delayed-enhancement scanning. The variation of cardiac morphology, motion, signal intensity of first-phase perfusion and delayed-enhancement were evaluated. Results There were 12 cases with ischemic myocardial disease in 23 cases and a total of 204 segments were analysed, of which 63(30.9%) showed abnormal wall motion. In abnormal wall motion segments, the segments of signal decreasing, normal and delayed-enhancement were 52,11,and 59 respectively. Systolic thickening ratio decreased in all segments of abnormal wall motion. Delayed en- hancement lesions were observed in the cases with hypertrophic cardiomyopathy(2 cases), dilated cardiomyopathy(1 case) and cardiac tumor(1 case). Conclusion The multimodality MR imaging can assess the myocardial lesions accurately and play an important role in the clinic diagnose of the myocardial lesions.
