摘 要:目的评价三维增强磁共振血管成像(3D CE-MRA)和CT血管成像(CTA)技术在下肢动脉狭窄性病变的应用价值。方法对195例疑有下肢动脉狭窄患者,49例行3D CE-MRA检查,其中10例行数字减影血管造影(DSA)检查;149例行CTA检查,其中20例行DSA检查。将每条下肢动脉分9个节段,分别对2组病例以DSA为金标准进行效果评价,对比2种检查方法差异。结果①3D CE-MRA与DSA对比,准确率94.4%,敏感性100%,特异性100%,2种方法差异无统计学意义(χ2=0.262,P>0.05)。②CTA与DSA对比,准确率96.4%,敏感性100%,特异性100%,其中Ⅱ级以上狭窄准确率94.9%,敏感性100%,特异性97.6%,2种方法差异无统计学意义(χ2=0.194,P>0.05)。结论3D CE-MRA与CTA在下肢动脉狭窄性病变诊断中均具有较高价值,二者能准确评价下肢动脉狭窄性病变,是可靠、无创的检查方法。Objective To evaluate the clinical value of three-dimensional contrast enhanced magnetic resonance angiography (3D CE-MRA) and CT angiography (CTA) in arteriostenosis disease of lower extremity. Methods One hundred and ninety five patients suspected of having lower extremity arteriostenosis disease, among these patients:49 patients underwent 3D CE-MRA, 10 case carried out digital suhstraetion angiography (DSA); 149 patients underwent CTA, 20 case carried out DSA. Every lower extremity was divided into 9 vascular segments, to contrast the difference between 2 groups using DSA as gold standard. Results (1) Comparing with DSA, the accuracy rate, sensibility and specificity of 3D CE-MRA were 94.4%, 100% and 100%. There was not significant difference between the 2 methods (×2= 0.262, P〉0.05). (2) Comparing with DSA, the accuracy rate,sensibility and specificity of CTA were 96.4%, 100% and 100%. There was not significant difference between the 2 methods (×2=0.194, P〉0.05). Conclusion Both 3D CE-MRA and CTA are good at evaluating the lower extremity arteriostenosis disease, they can evaluate lower extremity arterio- stenosis disease exactly. Both of these methods are reliable and non-invasive techniques.
