磷酸肌酸对老年患者重症呼吸机相关性肺炎免疫功能的影响

摘　　要：目的探讨磷酸肌酸对ICU重症呼吸机相关性肺炎老年患者细胞免疫功能的影响。方法 32例重症呼吸机相关性肺炎患者随机分成2组各16例,治疗组(CP组)给予磷酸肌酸治疗,对照组给予常规治疗;监测两组用药前后外周静脉血T淋巴细胞亚群、痰培养结果及ICU机械通气、住院天数及病死率。结果治疗组使用磷酸肌酸后,CD3+、CD4+、CD4+/CD8+、NK细胞百分率,用药当天就有升高,逐渐随用药时间延长较用药前明显升高,第7天达高峰,与治疗前和对照组比较差异均有统计学意义(P<0.05),治疗组ICU住院平均时间及机械通气时间,死亡率均有下降,与对照组比较差异有统计学意义(P<0.05)。结论临床上可考虑应用磷酸肌酸辅助治疗老年重症呼吸机相关性肺炎,能够增强机体的免疫防御作用,提高抗菌药物的疗效,对改善预后和生存率有一定作用。Objective To analyze of creatine phosphate on the ICU ventilator-associated pneumonia in critically ill elderly patients with cellular immune function.Methods 32 patients with severe ventilator-associated pneumonia were randomly divided into 2 groups of 16 patients.The treatment group received treatment of creatine phosphate（CP group） and the control group received conventional therapy.The peripheral blood T-lymphocyte subsets inspection,sputum culture results and ICU mechanical ventilation,length of stay and mortality were observed before and after treatment.Results The treatment group after using creatine phosphate,CD3＋,CD4＋,CD4＋/CD8＋ and NK cell percentage increased gradually with the prolonged treatment,which were significantly higher than before treatment and that of the control group（P〈0.05）.The average time length of ICU stay and mechanical ventilation and the mortality rate decreased,compared with the control group（P〈0.05）.Conclusion The clinical application could be considered on the adjuvant treatment of elderly patients with severe creatine phosphate ventilator-associated pneumonia,can enhance the body’s immune defense,and improve the efficacy of antimicrobial drugs,to improve the prognosis and survival rates have a role.
