超声引导在经皮肾镜联合气压弹道碎石取石术中的临床应用

摘　　要：目的研究超声引导在经皮肾镜气压弹道碎石取石术的临床应用价值。方法回顾性分析108例肾结石患者行超声引导下穿刺建立经皮肾通道联合气压弹道碎石取石术的临床资料。结果 108例均Ⅰ期成功建立经皮肾通道,95.37%的患者Ⅰ期结石取净,碎石手术时间为45～150min,平均住院时间为10.5d,5例行Ⅱ期碎石术。3例患者术后发生感染性休克,经抗休克治疗后缓解;5例患者术后出血,行超选择性肾动脉栓塞术。结论采用超声引导穿刺建立经皮肾通道联合经皮肾镜气压弹道碎石取石治疗肾结石安全、可靠,可在临床推广应用。Objective To evaluate the clinical application of ultrasound-guided percutaneous nephrolithotomy（PCNL）.Methods 108 patients with upper ureteral stones treated with the PCNL were analyzed.The procedures were performed under the color doppler ultrasound guidance.Results The passageway was successfully built in all patients in the first-stage operation,and the overall stone-free rate was about 95.37%.Operating time was about 45~150 minutes and average days of inpatient were 10.5 days.After the operations,there were a total of 5 patients with residual stones and three patients had infectious shock.Delayed massive hemorrhage in the kidney was detected in 5 patients after their surgeries.Conclusion Ultrasound-guided PCNL is a safe,minimally invasive and effective method for treating the renal calculus.
