摘 要:目的探讨痰结核分枝杆菌噬菌体生物扩增法(tuberculosis bacterium phage bioamplification,TB-PhaB)与痰结核菌脱氧核糖核酸(tuberculosis bacterium dcoxyribosc nucleic acid,TB-DNA)联合检测对痰菌阴性肺结核诊断的价值。方法选取初治痰菌阴性肺结核患者132例,进行TB-PhaB与TB-DNA检测。结果对于痰菌阴性肺结核,TB-PhaB检测的敏感度是51.52%、特异度是97.37%;TB-DNA检测的敏感度是62.88%、特异度是90.79%;TB-PhaB与TB-DNA并联联合检测的敏感度是82.58%、特异度是89.40%。结论对于痰菌阴性肺结核,TB-DNA或TB-PhaB单独应用容易漏诊;TB-PhaB与TB-DNA联合检测可为痰菌阴性肺结核的早期诊断提供重要的临床参考价值。Objective To investigate the diagnostic value of associated detection of phage bioamplification (phaB) and tuberculosis bacterium deoxyribose nucleic acid ( TB - DNA) in pulmonary tuberculosis with negative sputum smear and culture. Methods A total of 132 cases of initial treated pulmonary tuberculosis with negative sputum were detected with PhaB and TB - DNA. Results For the sputum negative pulmonary tuberculosis, the sensitivity of TB - PhaB detection was 51.52%, specificity was 97.37% ;the sensitivity of TB - DNA detection was 62.88%, specificity was 90.79% ;the sensitivity of TB - PhaB and TB - DNA combined detection was 82.58%, specificity was 89.40%. Conclusion For the sputum negative pulmonary tuberculosis, single application of TB - PhaB or TB - DNA can easily result in misdiagnosis. TB - PhaB and TB - DNA combined detection proves valuable in providing important clinical reference for the early diagnosis of sputum negative pulmonary tuberculosis.
