耳内镜下自体脂肪移植在鼓膜穿孔修补中的疗效观察

摘　　要：目的探讨耳内镜下自体脂肪移植在鼓膜穿孔修补中的疗效。方法选取2007年1月-2010年10月鼓膜穿孔患者68例为研究对象,随机分为对照组(颞肌筋膜修补术组)34例和观察组(耳内镜下自体脂肪移植修补术组)34例,对2组患者不同穿孔治疗总有效率、听力改善幅度、愈合时间、并发症发生率及治疗前后的血清转化生长因子(transformation growth factor,TGF)-α、TGF-β_1、TGF-β_2、白细胞介素-6(interleukin-6,IL-6)、C反应蛋白(C reaction protein,CRP)水平进行统计及比较。结果观察组的治疗总有效率高于对照组,听力改善幅度大于对照组,愈合时间短于对照组,并发症发生率低于对照组,治疗后血清TGF-α、TGF-β_1、TGF-β_2、IL-6、CRP水半低于对照组,差异有统计学意义(P<0.05)。结论耳内镜下自体脂肪移植在鼓膜穿孔修补中的效果要好于颞肌筋膜修补术,可显著改善综合临床效果,组织修复过程也较快。Objective To study the effect of endoscopic myringoplasty with autogenous adipose tissue in tympanic membrane perforation. Methods Sixty - eight patients with tympanic membrane perforation from January 2007 to October 2010 were selected as research object, who were randomly divided into control group （temporal fascia repair surgery group, 34 cases ） and observation group （endoscopic myringoplasty with autogenous adipose tissue repair surgery group, 34 cases）, then the total effective rate of different tympanic membrane perforation , improvement range of heating, healing time, incidence of complications, serum transformation growth factor （TGF） -α, TGF- β1, TGF- β2, interleukin-6（IL-6） and C reaction protein（CRP） before and after the treatment of two groups were analyzed and compared. Results The total effective rate of different tympanic membrane perforation of observation group were all higher than those of control group, serum TGF - α, TGF - β1, TGF - β2, IL - 6 and CRP were lower than those of control group, there were all significant differences （ P 〈 0.05 ）. Conclusion The effect of endoscopic myringoplasty with autogenous adipose tissue in tympanic membrane perforation is better than temporal fascia repair surgery, and it can significantly improve the comprehensive effect, and the tissue repair is faster.
