和营方联合激光治疗糖尿病视网膜病变临床研究

摘　　要：目的观察和营方联合激光治疗糖尿病视网膜病变的临床疗效。方法将69例病例随机分为治疗组和对照组,对照组34例65眼予激光光凝治疗,治疗组35例67眼予激光光凝治疗及口服和营方;两组均治疗12周,观察临床疗效及中医证候疗效、视力及眼底检查的变化情况。结果①治疗组、对照组临床疗效总有效率分别为79.1%、61.5%;组间临床疗效比较,差异有统计学意义(P<0.05)。②治疗组治疗前视力0.27±0.16,治疗后视力0.47±0.26;对照组治疗前视力0.26±0.15,治疗后视力0.40±0.24。两组组内治疗前后比较,差异有统计学意义(P<0.01);组间治疗后比较,差异具有统计学意义(P<0.05)。③两组治疗后眼底出血、渗出、黄斑水肿情况比较,差异有统计学意义(P<0.05,P<0.01),血管瘤、新生血管差异无统计学意义(P>0.05)。④治疗组、对照组中医证候总有效率分别为85.7%、67.6%;组间中医证候疗效比较,差异有统计学意义(P<0.01)。结论和营方联合激光治疗糖尿病视网膜病变,可有效缓解患者症状、提高视力、改善眼底病变,临床疗效良好。Objective To observe the clinical efficacy of ＂Heying Prescription＂ combined laser therapy in treating diabetic retinopathy. Methods Sixty-nine cases were randomized into two groups： control group in which 34 cases were treated by laser therapy, and treatment group in which 35 cases were treated by combination of laser therapy and modified ＂Heying Prescription＂, with the course of 12 weeks. The clinical efficacy, symptom score, eyesight and eye ground were detected, Results ① The overall effective rate was 79.1% in treatment group and 61.5 % in control group, with a difference between the two gruups （ P 〈 0.05 ）. ② In treatment group, the visual acuity was 0.27 ± 0.16 before treatment, and 0.47 ± 0.26 after treatment; in control group, the visual acuity was 0.26 ±0.15 betbre treatment, and 0.40 ±0.24 after treatment, with significant difference between before and ＇after treatment in each group （ P 〈 0.01 ） ; after treatment, there was significant difference between the two groups （ P 〈 0.05）.③ There were significant differences in fundus hemorrhage, effusion and edema of yellow spot between the two groups （ P 〈 0.05, P 〈 0.01 ） , and no significant difference was found in hemangioma and neovascularization （ P 〉 0, 05 ）. （4） The effective rate of TCM syndrome was 85.7% in treatment group and 67.6% in control group, with a difference between the two groups （P 〈 0.01 ）. Conclusion Combined ＂ Heying Prescription＂ and laser therapy can alleviate clinical symptoms, improve the eye sight and eye ground effectively for diabetic retinopathy.
