摘  要:目的观察前B细胞克隆增强因子(PBEF)在体外循环(CPB)术后大鼠肺组织中的表达。方法建立SD大鼠深低温停循环(DHCA)肺损伤模型,32只大鼠随机分为Sham组和DHCA15min、30min、45min组,术后6h处死大鼠留取血液和肺组织标本。酶联免疫吸附试验(ELISA)检测肺组织和血清PBEF表达及肺组织肿瘤坏死因子(TNF)-α和白细胞介素(IL)-1β含量,免疫组织化学法检测PBEF在肺组织中表达及定位;观察肺脏病理形态学改变;术中监测血气分析、血流动力学指标。结果DHCA15min、30min、45min组血清中PBEF表达分别为(6.636±0.124、8.663±0.129、11.067±0.210)、肺组织中分别为(7.502±0.648、15.269±0.803、24.311±0.524)较Sham组血清中(3.370±0.010)、肺组织中(4.664±0.006)明显升高(P〈0.01);免疫组织化学法发现肺血管内皮、肺泡上皮和中性粒细胞中PBEF均有表达;苏木素-伊红(HE)染色镜下观察可见随着DHCA时间的延长,肺损伤程度明显增加。结论随着体外循环肺损伤的加重PBEF表达增加,PBEF可能成为研究体外循环术后肺损伤新的切入点。Objective To observe the expression of pre-B-cell colony enhancing factor (PBEF) in lung injury after cardiopulmonary bypass in a rat model. Methods A rat model of deep hypothermia and circulatory arrest was established. Thirty-two male Sprague-Dawley rats, weighing 400-450 g, were ran- domly divided into four groups: sham group and DHCA 15 min, 30 min, 45 min groups. At 6th h post DHCA, the level of PBEF in plasma and lung tissue was measured by enzyme-linked immunosorbent assay (ELISA) , and the concentration of tumor necrosis factor (TNF)-α and interleukin (IL)-1β in lung tissue by ELISA. Lung samples were collected for hematoxylin and eosin (HE) staining and immunohistochemis- try. Blood samples were collected for blood gas analysis. Besides, hemodynamics was monitored. Results In DHCA 15 rain, 30 min, and 45 min groups, the expression of PBEF in plasma (6. 636 ±0. 124, 8. 663 ± 0. 129, 11. 067 ±0. 210) and lung tissue (7. 502 ±0. 648, 15. 269 ±0. 803, 24. 311±0. 524) were sig- nificantly higher than in sham group (PBEF in plasma: 3. 370±0. 010, and lung tissue: 4. 664 ±0. 006; P 〈 0. 01 ). Pathological examination of lungs revealed that more alveolar hemorrhage and neutrophil accu- mulation were observed in the DHCA group than in the sham group. At the same time, immunohistochemistry of lungs revealed that PBEF was observed in puhnonary vascular endothelial cells, alveolar epithelial cells and neutrophils. Conclusion The increased expression of PBEF in the lung associated with lung inju- ry was induced by cardiopulmonary bypass. PBEF may serve as a novel role in the pathogenesis of lung in- jury induced by cardiopulmonary bypass.
