新疆维吾尔族类风湿关节炎与HLA-DR4基因的相关性研究

摘　　要：目的探讨新疆维吾尔族类风湿关节炎与HLA—DR4基因的相关性。方法采用特异性引物一聚合酶链反应（PCR—SSP）对新疆维吾尔族157例类风湿关节炎患者和118例健康人的HLA—DR4基因进行检测。结果HLA—DR4基因在维吾尔族类风湿关节炎患者和健康人群中基因频率分别为41．4％和28．8％，两组比较差异有统计学意义（P〈0．05）。在类风湿关节炎患者中，HLA．DR4基因阳性组与阴性组在性别、发病年龄比较差异无统计学意义（P〉0．05），将病程分为≤2年和〉2年组，差异有统计学意义（P〈0．01）；HLA—DR4基因阳性组与阴性组在血沉、C反应蛋白、抗CCP抗体比较差异无统计学意义（P〉0．05），两组类风湿因子比较差异有统计学意义（P〈0．05），HLA—DR4阳性组与阴性组在晨僵、关节压痛数、关节肿胀数、双手x线片骨破坏等方面比较差异均无统计学意义（P〉0．05）。结论HLA—DR4基因是新疆维吾尔族人群类风湿关节炎的易感基因，与国内汉族类风湿关节炎患者HLA—DR4阳性率34％～62％报道结果基本一致。Objective To analyze the HLA-DR4 gene associations with RA in the Uygur nation- ality of Xinjiang region. Methods The methods of DNA amplification with sequence specific primers-pol- ymerase chain reaction（PCR-SSP） was used to determine the ttLA-DR4 gene in 157 RA patients and 118 heahhy controls from the Uygur nationality of Xinjiang region. Results The frequency of HLA-DR4 was 41.4% in Uygur nationality and 28.8% in healthy control group of Xinjiang region（P 〈0.05）. There is no statistically significant difference in sex,age of onset between the DR4 ＋ and DR4- in RA patients（P 〉0.05 ） ,the diffcrence was significant in duration of disease between ≤2 years and 〉2 years in RA pa- tients （P 〈 0.01 ） ;There is no difference in ESR, CRP, anti-CCP between the DR4 ＋ and DR4- in RA pa- tients（ P 〉 0.05 ） , the difference was signifcant in RF（ P 〈 0.05 ） ;There is no difference in morning stiff- ness,the pain number of joints,the swell numher of joints and X-ray about osteoclasia（P 〉 0.05）. Con- clusion HLA-DR4 gene is the p,＇edisposing gene for RA in the Xinjiang Uygur patients. Its positive rate is ahnost consistent with the reported results for Chinese Han RA patients（34%-62% ）.
