摘 要:目的调查和评估初发脑卒中患者延续性护理需求,以制订相应的健康教育对策。方法采用脑卒中患者延续性护理需求调查表对102例初发脑卒中患者在出院时进行调查。根据调查的结果,提出个体化和循证的健康教育对策。结果各项延续性护理需求率均在65%以上,最高达94.12%。根据Likert5级评分法,102例初发脑卒中患者延续性护理需求得分为(4.35±1.04)分,介乎于"比较需要"和"非常需要"水平之间。结论初发脑卒中患者对延续性护理有多方面的需求且需求程度高,加强社区和居家患者的健康教育,进行多学科协作、个体化、动态性的延续性护理是患者和家属迫切需要的。Objective To investigate the transitional care needs of first new-onset stroke patients,and on that basis,propose targeted and evidence-based health education strategies. Methods Transitional Care Needs of Stroke Patient Questionnaires were conducted in 102 first-onset stroke patients when they were discharged. Targeted and evidence-based health education strategies were proposed then. Results There are at least 65% percent patients in need of transitional care,even up to 94.12% percentage. Average score of the care need was 4.35+1.04 in the 102 cases,and the level of need was between extreme need and much more need. Conclusion The first new-onset stroke patients need nursing guidance very. much,and individual,dynamic,and continuous care should be provided.
