摘　　要：目的了解新疆美沙酮门诊的运行成本,为当地艾滋病防治经费的合理分配提供科学依据。方法回顾性查阅财务及业务资料,收集与美沙酮门诊实施相关的直接成本及政府支持性成本;查阅美沙酮网络直报数据库,获得美沙酮维持治疗覆盖人数。结果美沙酮门诊运行早期,医院管理模式总成本高于疾控管理模式,其中疾控管理模式的政府支持性成本更高,医院管理模式的直接成本更高,但随着门诊的持续运行,总成本接近一致。美沙酮门诊成本构成主要为人力资源及政府支持性成本,其中政府支持性成本主要用于当地工作人员的工资投入、当地政府对美沙酮门诊房屋的投入,以及美沙酮药物的购买、配液及运输费用。美沙酮门诊每成功纳入1人的总成本为1 578～2 584元,每成功维持1人的总成本为3 262～4 061元/年。结论美沙酮门诊运行早期医院管理模式的成本高于疾控管理模式;以目前获得的美沙酮门诊人均总成本进行估算,国家对美沙酮门诊的投入已超过其实际需求;将一名海洛因滥用者稳定在美沙酮门诊的总成本远高于将其成功纳入的总成本,且两者之间的比值呈逐年下降趋势。Objective To conduct cost analysis of MMT in Urumqi, Xinjiang, so as to provide technical support for the reasonable distribution of local HIV/AIDS prevention and control budget. Methods The direct cost and government support cost were collected through the review of financial and technical materials. The coverage of MMT was also collected through MMT reporting system. Results The total cost of hospital based clinic was higher than the CDC based clinic in early time. There was higher government input for CDC based clinic and higher direct cost for hospital based clinic. The results of cost composition analysis showed that the maintaining cost of MMT were for human resource and government input which was used to provide salary for local workers, house rent buy and distribute methadone. The results of average cost analysis showed that the cost for recruiting one drug user successfully was from 1 578 to 2 584 Renminbi yuan （RMB）, the cost for maintaining one person in MMT was from 3 262 to 4 061 RMB per year. Conclusion.. The cost of hospital based clinic is higher than that of the CDC based clinic in early time. The MMT seems higher than the real needs. The average maintaining cost is higher than the recruiting cost, but the ratio between them is reduced in recent years.
