摘 要:目的探讨慢性充血性心力衰竭(CHF)患者血清生长分化因子-15(GDF-15)水平的变化,并探讨其临床意义。方法选择CHF患者60例,测定血清GDF-15水平变化,并与30名健康对对照组进行比较。结果 CHD患者左室收缩末期内径(LVESD)、左室舒张末期内径(LVEDD)、心胸比值和血清GDF-15明显高于健康对照组,左室射血分数(LVEF)明显低于健康对照组(P<0.01);随着NYHA分级的增高,CHF患者血清GDF-15水平相应提高(P<0.05或0.01);血清GDF-15和LVEF呈负相关(γ=-0.658,P<0.01),和LVEDD、LVESD和心胸比值呈正相关(γ=0.588,0.596,0.621,P<0.01)。结论血清GDF-15可反映心功能的状态,并对CHF的诊断和病情判定有重要意义,有望成为CHF的新生标志物。Objective To investigate the changes and clinical significance of serum levels of growth differentiation factor--15(GDF--15) in patients with chronic congestive heart failure. Methods Total of 60 patients with chronic congestive heart failure were matched with 30 healthy controls and the serum levels changes of GDF--15 were detected. Results The left ventrieular end systolic diameter(LVESD), left ventrieular end diastolic diameter(LVEDD), cardiothoracic ratio and serum GDF--15 in researched groups was higher than the controls and the left ventricular ejection fraction(LVEF) was lower than the controls (P 〈 0.01) ; The serum levels of GDF--15 in patients with CHF rose as the NYHA classifica- tion went up and it had statistically significance (P 〈 0.05 or 0.01) ; the serum levels of GDF--15 were significant negative correlation with LVEF (r=-0. 658, P 〈 0.01) and was significant positive correla- tion with LVEDD, LVESD and cardiothoracic ratio (r = 0. 588, 0. 596, 0. 621, P 〈 0.01). Conclusions Serum levels of GDF--15 may serve as a good biomarker for reflecting cardiac status and as an important biomarker to diagnose and determine the state of CHF. It may he a new marker for CHF.
