CIK细胞治疗中晚期食管癌的疗效观察及护理

摘　　要：目的总结CIK细胞免疫治疗中晚期食管癌的效果和护理经验。方法 2009年10月～2010年10月间36例中晚期食管癌患者给予CIK细胞免疫治疗,治疗前、中、后给予全面的护理并记录不良反应。治疗结束1周后查T淋巴细胞亚群及NK细胞群,四个疗程后随访观察瘤体、生活质量的变化。结果 36例患者中,客观缓解率为44.4%。KPS评分增加≥10分为24例(66.7%),10例评分无变化,但食欲及睡眠均有改善。2例评分下降10分。复查T淋巴细胞亚群及NK细胞群,提示全部病例细胞免疫功能均有所提高。治疗期间5例出现发热(体温37.5℃～38.5℃),1例出现皮疹,经对症处理后治愈。结论 CIK细胞免疫治疗是中晚期食管癌的有效手段,能够提高患者的免疫功能及改善生活质量。在治疗过程中,全面、细致、规范的护理是保证治疗取得成功的前提。Objective To summarize the therapeutic effect and nursing experience of treating esophageal cancer in middle stage or advanced stage with immunotherapy of CIK cell. Methods Selected 36 cases of esophageal cancer in middle stage or advanced stage who accepted CIK cell treatment from October,2009 to October,2010 and patients were provided nursing care and the untoward reactions were recorded. One week after treatment T-lymphocyte subsets and NK cells were detected,after 4 courses of treatment patients were followed up to observe the change of tumor mass and life quality. Results The objective remission rate among 36 patients was 44.4%. There were 24 patients whose KPS score increased more than 10 points （66.7%）,10 patients＇ score had no change while their appetite and sleep were improved. However,2 patients＇ score decreased about 10 points. The results of detecting T-lymphocyte subsets and NK cells showed that immune function of all the patients was improved in different extent. During the treatment course,5 patients had fever （37.5 ℃-38.5℃ ）,one had skin rash. All symptoms recovered after proper treatment. Conclusion Immunotherapy with CIK cell is an effective method for esophageal cancer and able to improve immune function and life quality of patients. In addition,in the process of treatment,complete,cautious and normative nursing care is the precondition to make sure the treatment successful.
