刮吸手术解剖法在复杂胆道手术中的应用体会

摘　　要：目的总结在胆道手术中运用刮吸解剖法的手术治疗经验。方法 2007年1月至2011年12月,我科应用刮吸解剖法施行急诊胆囊切除术72例,肝十二指肠骨骼化清扫手术110例。结果 182例手术均获成功,无胆管损伤、胆漏、术后出血等并发症。肝十二指肠韧带骨骼化清扫手术中未出现胆管及血管的严重损失。结论应用刮吸解剖法施行胆道手术能缩短手术时间,操作更加精细,可减少术中出血,增加手术安全性。此法有助于肝门结构显露,增强了肝十二指肠韧带骨骼化程度,提高了根治性切除率。刮吸解剖法是一种实用、简便且安全的手术操作方法。。Objective To summarize the experience of the application of curettage and aspiration technique（CAT） in operative dissection of involved biliary-tract surgery. Methods A retrospective analysis was made on patients underwent operation from 2007,1- 2011,12 in our department, Application of curettage and aspiration technique with emergency cholecystectomy on 72 cases, gallbladder Carcinoma and bile duct Carcinoma with skeletonizationof Hepatoduodenal ligament were done on 1 l0 cases. Results 182 cases have been successfully. Complcations as Bile duct injury, postoperative bile leakage , bleeding and other complications do not appear. Injury of bile ducts and blood vessels do not appear in skeletonizationof Hepatoduodenal ligament. Conclusion CAT as a method for operation is very helpful to expose operative field, increase the skeletonzation of liver-duodenum and increase the radical resection rate and survival rate. It is a practical simple and safe method for biliary-tract surgery.
