长期低剂量罗红霉素治疗难治性鼻及鼻窦炎的临床观察

摘　　要：目的 观察长期低剂量罗红霉素治疗难治性鼻及鼻窦炎（RRS）的临床疗效.方法 52例RRS患者随机分成两组,对照组治疗上给予常规鼻腔冲洗、局部类固醇激素喷鼻、黏液促排剂、术腔定期清理等治疗.观察组在对照组治疗方法上,再给予罗红霉素50mg,1次/d,连续服用6个月.两组分别记录治疗后6个月时鼻塞、流涕、头痛、嗅觉缓解等四项指标,由患者用0~100分对主观临床症状进行评定.同时两组于治疗前后行鼻窦CT检查,并采用Lund-Mackay鼻窦CT评分法进行评分.结果 随访6个月,两组各项症状分值及鼻窦CT评分疗效比较,结果两种疗效评估方法皆显示两组间差异有明显统计学意义（P〈 0.05）,具有一致性.结论 长期低剂量使用罗红霉素治疗RRS有较好临床疗效.Objective To observe the clinical effect of the treatment in in intractable rhinitis and nasosinusitis （RRS） with low-dose roxithromycin for a long term. Methods 52 patients with RRS were divided into two groups randomly. The control group was given the treatment of routine swill in the nasal cavity, spurting the nasal cavity with steroid hormone partly, the mucus eccritic and the periodic cleaning of the nasal cavity. With the same treatment as the control group, futhermore, the observation group was given 50mg roxithromycin one time a day for 6 months. Both the two groups were recorded with the 4 indicators including nasal congestion, runny nose, headache and sense of smell remission respectively 6 months after treatment, evaluated by the patients from 0-100 points by subjective clinical symptoms. Both the two groups were given CT detection of sinus before and after the treatment, marked by Lund- Mackay CT detection of sinus scoring method. Results During 6 months follow up period, comparing the two groups in the scores of symptoms and CT detection of sinus, It showed that there was manifest statistical significance in both the effect evaluation methods. Conclusion There is fairly good clinical effect to treat RRS with low-dose roxithromycin for a long term.
