子宫全切术后盆底功能障碍与时间相关性的研究

摘　　要：目的通过对子宫全切术后不同时期的患者进行盆底康复锻炼的评估,了解子宫全切术后盆底肌肉功能障碍与时间的相关性。方法将2006年6月-2008年6月、2010年6月-2010年9月、2011年3月-2011年6月和2011年6月-2011年9月的子宫全切术后未绝经的患者分别为A组、B组、C组和D组,检测患者盆底肌力、盆底肌电峰值和对其进行问卷调查。结果⑴B、C、D组盆底肌力和肌电峰值明显高于A组(P<0.01);C、D组间无差异(P>0.05);B组比C、D组明显降低(P<0.01)。⑵B、C、D组盆底功能障碍调查表的分值明显高于A组(P<0.01)。⑶B组和C组的性功能调查表的分值间无差异(P>0.05),A和B组、A组和C组、C组和D组、B组和D组间有差异(P<0.05)。结论⑴不同时期子宫切除术后的患者盆底肌力、肌电峰值的变化,提示子宫切除术能引起盆底功能障碍;随着时间的延长,在一定程度上,子宫全切术后盆底肌力及肌电峰值均逐渐下降。⑵盆底肌力和肌电峰值检测可作为提示盆底功能障碍的检查手段。Objective To evaluation and treatment the different period of patients by pelvic floor dysfunction after total hysterectomy, understand correlation of time and symptom of pelvic floor dysfunction. Methods Divicte all the different period of the patients respectively into 4 groups as group A, group B, group C and group D, to detection the four groups of patients at the muscle strength, and pelvic floor muscles electricity peak, then surveyed them by pelvic floor distress inventory-short form 20 and prolapse/urinary incontinence sexual questionnaire 12. Results （1）In pelvic floor muscles strength and pelvic floor muscles electric Peak of four groups, group B, C, D was significantly higher than the group A（P〈0.05）; the muscle strength of group B is lower than group C, D（P〈0.05）. （2）In pelvic dysfunction questionnaire, group B, C, D was significantly higher than group A（P〈0.05）; among groups B, C, D are differences（P〈0.05）. （3）In sexual questionnaire, group B and C is no statistical difference（P〉0.05）, group A and B, group A and C, group C and D, group B and D are statistically significant difference（p〈0.05）. Concltmion The Patients＇ muscle strength or pelvic floor muscles electricity peak are difference, hysterectomy can cause pelvic dysfunction, detection the pelvic floor muscle strength and museles electric peak can be used as a tip of pelvic floor dysfunction. To a certain degree, there is a positively correlation between symptoms of pelvic floor dysfunction and the different period of patients after total hysterectomy.
