改良Miccoli微创手术治疗良性甲状腺疾病的疗效观察

摘　　要：目的探讨改良Miccoli微创手术治疗良性甲状腺疾病的临床疗效。方法良性甲状腺疾病患者88例随机分为微创组52例和对照组36例,微创组采用改良Miccoli微创手术治疗,对照组采用开放性手术治疗。比较2组术中、术后情况及并发症情况。结果微创组手术时间长于对照组,住院费用多于对照组,术中出血量、术后引流量、切口长度、术后住院天数均小于对照组,差异有统计学意义(P<0.01)。2组并发症发生率比较,差异无统计学意义(P>0.05)。结论甲状腺改良Miccoli微创手术具有切口美观,手术创伤小,术后恢复快等优点,但其手术时间相对较长,住院费用较高。临床上应根据实际情况及患者意愿决定手术方式。Objective To discuss the clinical efficacy of improved Miccoli minimally invasive thyroidectomy in treatment of benign thyroid disease.Methods 88 cases of patients with benign thyroid diseases were randomly divided into minimally invasive group of 52 cases and control group of 36 cases.The minimally invasive group was treated by improved Miccoli minimally invasive thyroidectomy,while the control group was treated by open thyroidectomy.The intraoperative and postoperative situation and complication of 2 groups were compared.Results Operating time of minimally invasive group were longer than control group;Hospitalization expenses of minimally invasive group was higher than control group;Peri-operative bleeding,total volume of drainage of minimally invasive group after operation were less than that of the control group;Incision length and hospital stays of minimally invasive group after operation was shorter than that of the control group;and the differences were statistically significant（P〈0.01）.Complications number of 2 groups had no significant difference（P〈0.05）.Conclusion Improved Miccoli minimally invasive thyroidectomy can shorten incision length,decrease peri-operative bleeding and total volume of drainage after operation,shorten hospital stays,but shows longer operating time and higher hospitalization expenses.The doctor should choose operation method according to the actual situation and patients will clinically.
