摘  要:目的通过建立2次打击大鼠致多器官功能障碍综合征(MODS)的动物模型,探讨黄芪对不同复苏液体复苏后MODS中血管内皮细胞损伤的影响。方法将32只SD大鼠随机分为对照组、乳酸林格液复苏组(RL组)、7.5%的高渗盐水和6%的羟乙基淀粉混合液复苏组(HTH组),HTH组中分为复苏后未注射黄芪组(HTH1组)和复苏后注射黄芪组(HTH2组)。RL组和HTH组复制大鼠MODS复苏的动物模型,HTH2组腹腔注入黄芪注射液,HTH1组和RL组注入0.9%氯化钠溶液。分别在注射黄芪注射液和0.9%氯化钠溶液后24 h采集血液和肺组织标本,测定大鼠血清中循环内皮细胞(CEC)数量、一氧化氮(NO)、内皮素-1(ET-1)、肺组织中肿瘤坏死因子-α(TNF-α)和细胞间黏附分子(ICAM-1)的含量、白细胞介素-6(IL-6)、血管细胞黏附分子(VCAM-1)mRNA,肺组织标本行光镜下病理检查。结果 HTH2组大鼠腹腔注射黄芪注射液后24 h CEC数量、NO、ET-1含量明显低于RL组、HTH1组(均为P<0.05);HTH2组大鼠腹腔注射黄芪注射液后24 h TNF-α、ICAM-1、IL-6、VCAM-1含量明显低于HTH1组(均为P<0.05)。结论黄芪注射液对不同复苏液体复苏后MODS中血管内皮细胞有明显的保护作用。Objective To investigate the influence of Huangqi Injection on vascular endothelial injury in multiple organ dysfunction syndrome (MODS) after anabiosis with different resuscitation fluid through establishing the model of MODS in rats with two hits. Methods SD rats (n = 32) were randomly divided into control group, RL group (resuscitated with Lactated Ringer's Solution), HTH group (resuscitated with 7.5% hypertonic saline and 6% hydroxyethyl starch mixture), and HTH group was divided again into HTH1 group (without Huangqi Injection after anabiosis) and HTH2 group (with Huangqi Injection after anabiosis). The animal model of MODS after anabiosis was copied in RL group and HTH group. HTH2 group was given intraperitoneal Huangqi Injection and HTH1 group and RL group was given 0.9% sodium chloride solution. After 24 hours the blood samples and lung tissue samples were collected for detecting the number of serum circulating endothelial cells ( CEC), and content of nitric oxide ( NO), endothelin-1 (ET-1), lung tissue tumor necrosis factor-or (TNF-α) and intercellular adhesion molecule-1 ( ICAM-1 ), interleukin-6 (IL-6), vascular cell adhesion molecule-1 ( VCAM-1 ) mRNA, and pathological changes of lung tissue were examined with light microscope. Results The number of CEC and content of NO and ET-1 were significantly lower in HTH2 group than those in RL group and HTH1 group ( all P 〈 0.05 ). The content of TNF-α, ICAM-1, IL-6 and VCAM-1 were significantly lower in HTH2 group than those in HTH1 group (all P 〈 0.05 ). Conclusion Huangqi Injection has obvious protective effect on vascular endothelial of MODS after anabiosis with different resuscitation fluid.
