急性右室梗塞住院病人的康复运动

摘　　要：方法 :观察了 2 0例急性右室梗塞住院病人康复运动方案的疗效 ,此 2 0例合并下壁、后壁或前壁心肌梗塞。心功能Ⅱ、Ⅲ级共 13例 ,12例伴有心律失常 ,16例血管造影发现右冠脉阻塞性病变。均参加 3阶段康复方案。结果 :13例完成 11次以上的住院康复运动 ,7例低于 11次。康复运动期间除 2例有偶发的房早外 ,无心绞痛和低血压或心律失常发生。心脏功能容量值与康复运动次数明显相关 (r=0 86 82 ,p <0 0 0 0 5 )。 13例心功能容量高于 3 0METs和 7例低于 3 0METs病人的冠脉损害程度、心功能等级和心律失常相似。结论 :住院期心脏康复运动方案是安全的 ,并能帮助医学状态较复杂的病人从低体能状态恢复。右室梗塞病人参加住院期康复运动方案对改善心功能是有益的。Methods The effects of rehabilitation exercise program (REP) were observed in 20 inpatients with acute right ventricular infarction (ARVI). The inpatients with ARVI complicated with inferior and posterior or anterior myocardial infarction . Cardiac function classⅡand Ⅲ were found in 13 inpatients, 12 inpatients complicated with arrhythmia. Angiography showed that the obstruction lesion of right coronary artery in 16 cases. All inpatients entered the stages 3 of REP.Results 13 inpatients completed over 11 sessions of inpatient rehabilitation exercise,7 inpatients completed less than 11 sessions of inpatient rehabilitation exercise. There were no symptom of angina and signs of lower blood pressure or arrhythmia during the period of REP except that 2 inpatients had occasionally atrial premature contraction. The values of cardiac function capacity were significantly correlated with the sessions of rehabilitation exercise training (r=0.8682,p<0 0005). The grade of coronary arteries lesion, cardiac function class and arrhythmia in 13 inpatients with cardiac function capacity over 3 0 METs were similar to those in 7 inpatients with cardiac function capacity below 3 0 METs.Conclusion The inpatient REP was safe and useful in helping patients with medical complications to recover from deconditioning situation at early stage. Patients with RVI could benefit for improvement of cardiac function if they participated in the inpatient REP.
