摘  要:目的评价慢性阻塞性肺疾病患者机械通气期间采用气道分级管理的干预效果。方法按患者入科顺序将65例患者分为观察组33例和对照组32例。对照组按常规气道管理,每2h予胸部物理治疗;观察组按气道评分实施气道分级管理,行不同频次物理治疗。比较两组机械通气第5天、第10天的潮气量、呼吸频率、动脉血二氧化碳分压及急性生理与慢性健康评分,比较两组机械通气时间和住院时间。结果机械通气第10天,观察组潮气量、呼吸频率、动脉血二氧化碳分压的监测值优于对照组,急性生理与慢性健康评分低于对照组,机械通气时间明显少于对照组,经比较,差异均有统计学意义;两组住院时间比较,差异无统计学意义。结论气道分级管理能有效改善慢性阻塞性肺疾病机械通气患者的呼吸功能。Objective To evaluate the intervention effect of airway management based on different grades applied in me chanically ventilated patients with chronic obstructive pulmonary disease(COPD). Method Divide 65 patients into observa- tion group (n=33) and control group (n=32) by order. Patients in control group receive routine airway management of chest physiotherapy every 2 hours. Patients in observation group receive airway management based on different grades of chest physiotherapy on different frequency according to airway scores. Compare the tidal volume, respiratory rate, PaCO2 and scores of acute physiology and chronic health of two groups on the 5th and 10th day undergoing mechanical ventilation. Compare the mechanical ventilation time and hospitalization time. Result Tidal volume, respiratory rate, PaCO2 and scores of acute physiology and chronic health are significantly better in observation group. Mechanical ventilation time is signifi- cantly shorter in observation group. There is no significant difference on hospitalization time between two groups. Conclu- sion Airway management based on different grades can improve the respiratory function for patients with chronic obstruc- tive pulmonary disease effectively.
