剖宫产术后再次妊娠阴道试产结局的影响因素分析及护理

摘　　要：目的探讨剖宫产术后再次妊娠采用阴道试产的影响因素。方法分析121例剖宫产术后妊娠阴道试产的资料,记录试产结果,拟定影响因素及赋值。应用Bayes判别法构建试产结局方程,采用回代法和刀切法来评价试产结果。结果 14个影响因素中距前次剖宫产时间、子宫下段厚度、年龄、新生儿出生体重、妊娠期高血压疾病5个因素保留在判别函数中;回代法验证试产成功和失败的正确率为84.1%和82.7%,刀切法验证为81.2%和80.8%。结论距前次剖宫产时间、子宫下段厚度、年龄、新生儿出生体重、妊娠期高血压疾病是阴道试产结局的重要影响因素;利用判别函数选择合适的试产对象,并加强护理,以减少剖宫产率。Objective To study the influencing factors of vaginal delivery in repeated pregnancy after cesarean section women. Method Analyze the information of 121 cases of vaginal delivery in repeated pregnancy after cesarean section. Re cord delivery results, assign influencing factors and valuation. Build up delivery result equation by Bayes Discriminant. E- valuate delivery result by back/forward sweep method and jackknife method. Result Five factors of time from the previous cesarean section, lower uterine segment thickness, age, infant birthweight and pregnancy hypertension from 14 influencing factors are maintained in discriminant function. Correct rate of success and failure checked by back/forward sweep method is 84. 1% and 82. 7%. Correct rate checked by jackknife method is 81.2% and 80. 8%. Conclusion Time from the previous cesarean section, lower uterine segment thickness, age, infant birthweight and pregnancy hypertension are important factors of vaginal delivery. Choose suitable delivery object by discriminant function and enhancing nursing care can reduce the rate of cesarean section.
