摘  要:目的探讨经阴道彩色多普勒超声诊断老年女性膀胱颈梗阻的临床价值。方法对20例经膀胱镜检查或/及手术证实的膀胱颈梗阻患者行经阴道彩色多普勒超声检查,观察尿道上段及膀胱颈前后唇的形态及回声,通过彩色多普勒超声观察病变与尿道的解剖关系,分别于纵切及横切时测量膀胱颈的横径、厚径及后唇厚度,测量残余尿量。结果所有受检者经阴道超声均可获得膀胱颈连续清晰的图像,所有患者膀胱颈横径均>20 mm,厚径均>15 mm,后唇厚>6 mm,膀胱残余尿量>50 mL,通过排尿期显示彩色尿流信号自该病变中通过。结论经阴道彩色多普勒超声检查能清晰显示尿道及膀胱颈结构,排尿期彩色尿流排除肿瘤病变,为该病提供了一种简便可行的诊断方法。Objective To investigate the clinical value of transvaginal color Doppler ultrasonography (TVCDUS)in the diagnosis of elderly female bladder neck obstruction. Methods Twenty female patients with bladder neck obstruction were examined by TVCDUS, the morphous and echo of their upper urethra, front lip, posterior lip of bladder neck were detected, and the anatomical relationship of lesions and urethra were observed by color Doppler flow imaging (CDFI). The transverse diameter, anteroposterior diameter, posterior lip thickness and residual urine volume were measured respectively when the transverse and longitudinal section scanning was being carried out. Results Continuous and clear images of bladder neck could be seen according to TVCDUS, and their bladder necks' transverse diameters were larger than 20 mm, anteroposterior diameters were larger than 15 mm, posterior lip thickness were larger than 6 mm, their vesical residual urine volume were larger thanS0 mL. The color flow signal of urinary passed the leisons during micturition period. Conclusion The TVCDUS can clearly display the structure of urethra and bladder neck,and rule out the possibilities of the neoplastic lesion by observing the color signal of color flow signal during micturition period. So, it provids a convenient diagnostic method for elderly female bladder neck obstruction.
