摘　　要：目的了解湖南省2009年出生缺陷的流行状况,探讨孕产妇分娩年龄与居住地对出生缺陷的影响。方法利用logistic回归分析方法,对湖南省2009年出生缺陷监测资料进行分析。结果湖南省2009年共监测出生缺陷儿1 856例,出生缺陷发生率为21.35‰;城镇儿童出生缺陷发生率（22.60‰）高于农村儿童（20.22‰）,OR值为1.120（95%CI for OR：1.022~1.228）;出生缺陷的发生率随孕产妇分娩年龄呈U形变化,＂〈20岁＂组、＂20-岁＂组、＂≥35岁＂组的发生率分别为77.72‰、20.32‰和23.86‰,各年龄组间比较具有统计学意义（χ2=182.128,P=0.000）;相对于＂20-岁＂组,低龄（〈20岁）和高龄（≥35岁）孕产妇出生缺陷的OR值分别为4.168（95%CI for OR：3.340～5.200）和1.184（95%CI for OR：1.003~1.397）。结论湖南省2009年出生缺陷发生率较往年有升高趋势;居住在城镇、低龄和高龄孕产妇均会增大出生缺陷的发生风险。[ Objective ] To descriptive the prevalence of birth defects in Hunan Province in 2009, and to seek after the effects of delivery age and habitat on birth defects. [ Methods ] Using logistic regression analysis to analyze the birth defects monitoring data. [Results] 1856 infants with birth defects were monitored in Hunan province in 2009, the prevalence rate was 21.35%v; In urban area the rate（22.60%v） was higher than that in rural area （20.22%v）, the odds ratio was 1.120 （95%CI for OR： 1.022-1.228）; The prevalence rate of birth defects assumed the ＂U＂ shape changes with maternal delivery age; The prevalence rate of the group of ＂〈20 years old＂, ＂20- years old＂ and ＂/〉35 years old＂ were 77.72%o ,20.32%0 and 23.86%0 respectively; Differences among each groups were statistics signifi- cant （X2=182.128, P =0.000）; Relative to the ＂20- years old＂ group, the OR values of younger delivery age （〈20 years old） group and elderly delivery age （≥35 years old） group were 4.168 （95%CI for OR： 3.340-5.200） and 1.184 （95%CI for OR： 1.003-1.397） respectively. [Conclusion] The prevalence of birth defects in Hunan province in 2009 was higher than the past years; Living in urban areas, younger delivery and elder delivery might increase the risk of birth defects.
